About this file

This is a Darfur Contracting Act Certification form for the State of California related to an informal competitive solicitation by the Westley SRRA for refrigeration equipment (two 54-inch two-section reach-in refrigerators and two 27.5-cubic-foot single-door reach-in freezers). Bidders and proposers with business activities or operations outside the United States within the previous three years are required to complete this certification form as a condition of bid eligibility under California Public Contract Code section 10478.

The certification requires companies to declare their international business status and confirm their eligibility to submit a bid by selecting one of three options: attesting they have no international business activities; disclosing they are a scrutinized company with written permission from the California Department of General Services to bid; or certifying they have international business activities but are not a scrutinized company as defined in Public Contract Code section 10476. For the third option, an authorized company representative must execute the certification under penalty of perjury, providing their printed name, title, signature, and the date and county/state of execution. The form requires insertion of the company name and Federal ID Number at the top.
