About this file
This is a Darfur Contracting Act Certification form required by the State of California for an informal competitive solicitation (06-0819D) involving vending machines. Bidders or proposers with business activities or operations outside the United States within the previous three years must complete this certification to establish eligibility. The solicitation seeks six snack vending machines and three cold drink vending machines.
The certification requires bidders to select one of three options: certify they have no international business activities, provide written permission from the Department of General Services if classified as a scrutinized company, or certify they have international operations but are not a scrutinized company as defined under Public Contract Code section 10476. Bidders must provide their company name, Federal ID Number, and authorized signature with printed name, title, and execution date and location. This certification is made under penalty of perjury under California law and is mandatory for bid submission eligibility.
