PERFORMANCE WORK STATEMENT (PWS)
1.0 GENERAL: This is a non-personal services contract to provide Non-Hazardous Solid Waste Collection, Removal, Disposal, and Management Services for the James A. Haley Veterans Hospital (JAHVH) located at 13000 Bruce B. Downs Blvd; Tampa FL 33612 and the Brooksville CBOC at Brooksville, FL.
A. The Contractor shall perform to the standards specified in this Performance Work Statement
(PWS)
B. The Government will not exercise any supervision or control over the contract service providers.
C. Contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.
1.1 DESCRIPTION OF SERVICES / INTRODUCTION: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and non-personal services necessary to perform all Non-Hazardous Solid Waste (Processable and Non-Processable) Collection, Removal, Disposal, and Management Service Tasks and Frequencies as described and defined in this Performance Work Statement (PWS). The Contractor shall perform to the standards provided in this PWS. Biomedical waste, sharps, hazardous materials are not included.
1.2 BACKGROUND: The James A. Haley Veterans Hospital (JAHVH) located at 13000 Bruce B.
Downs Blvd; Tampa FL 33612 has a 30 CYD Compactor, 30 CYD Open Containers, and 8 CYD Front End Loading (FEL) Containers for collection, removal, and disposal of solid waste, yard waste, and recyclables at various site locations. The Brooksville CBOC at Brooksville, FL has one (1) 8 YD Front End Loading (FEL) Container for collection and removal of solid waste. See Attachment 2, “Container List, Location, Site Map”.
1.3 OBJECTIVES: The Contractor shall ensure all work accomplished in accordance with this PWS meets all applicable Federal, State, and local laws, regulations, directives, instructions, manuals, and other requirements. Attachment 2, “Container List, Location, Site Map” provides the list of containers, identifies solid waste and recyclables collection stations, container type and size, type waste, tag no., container quantity, estimated service quantities, and Pick-up Frequencies.
1.4 SCOPE: The Contractor shall perform all Solid Waste Removal and Management Services in accordance with the PWS and PWS Attachments, including all applicable Federal, State, and local laws, regulations, directives, instructions, manuals, and other requirements. During performance of the contract, the Government reserves the right to relocate, combine, or eliminate pick-up stations and/or add/delete level of maintenance services required. A modification to the contract will be executed to incorporate these changes.
A. Services include: Scheduled solid waste collection, removal, disposal, and maintenance services (for all specified collection stations and equipment) and as ordered / requested specific tasks.
B. Services do NOT include: Collection of any type of waste other than non-hazardous solid waste and recyclables
PERFORMANCE WORK STATEMENT (PWS)
1.5 PLACE OF PERFORMANCE: The work to be performed under this requirement will be performed throughout the entire area occupied by the James A. Haley Veterans Hospital (JAHVH) located at 13000 Bruce B. Downs Blvd; Tampa FL 33612 and the Brooksville CBOC at Brooksville, FL. A site map of the JAHVH and a listing of containers and collection stations are provided in Attachment 2, “Container List, Location, Site Map”
1.6 PERIOD OF PERFORMANCE: The period of performance shall be for one (1) Base Year of 12 months and four (4) 12-month option years. The period of Performance shall read as follows:
Base Period, Option Period 1, Option Period 2, Option Period 3, Option Period 4
1.7 TYPE OF CONTRACT. The Veterans Administration will award a FIRM FIXED PRICE (FFP) Indefinite Quantity/Indefinite Delivery (IDIQ) type of contract with some “As Required” Contract Line Items (LIN’s) with estimated quantities.
1.8 GOVERNMENT ROLES AND RESPONSIBILITIES:
1.8.1 CONTRACTING OFFICER (CO): The CO is the only person authorized to make any changes in any of the requirements listed on this PWS; notwithstanding any provisions contained elsewhere in this contract, said authority remains solely with the CO. In the event the Contractor makes any change at the direction of any person other than the CO the change will be considered to have been made without authority and solely at the risk and expense of the Contractor.
A. All contract administration will be affected by the CO.
B. Communications pertaining to contractual administrative matters shall be addressed to the CO.
C. No changes in or deviations from the terms and conditions shall be affected without a written modification to the contract, executed by the CO authorizing such changes.
D. The CO will approve all Deliverables and Plans required in this PWS.
1.8.2 CONTRACTING OFFICER’S REPRESENTATIVE (COR): The CO will appoint a COR during the performance of this requirement. Additional Government personnel may be designated to assist in contract oversight. Other surveillance personnel may be designated as a COR and shall be trained and appointed in accordance with the Standard for Contracting Officer’s Representative (COR) Certification.
The COR will aid in identification and resolution of problems, conflicts in priority, subtask requirement definitions, and other operations type problems. The COR will perform IAW the responsibilities and duties identified in the COR designation letter.
A. COR AUTHORITY: A letter of designation will be issued to the COR by the CO. A copy of the letter will be sent to the Contractor. The letter of designation states the responsibilities and limitations of the COR, especially regarding changes in cost, price estimates, or changes in delivery dates. The COR is not authorized to change any of the terms and conditions regarding cost, quantity, or schedule of the resulting order.
B. COR FUNCTIONS: The COR shall monitor all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions:
1. Assure the Contractor performs the technical requirements of the contract.
2. Perform inspections necessary in connection with contract performance.

3. Maintain written and oral communications with the Contractor concerning technical aspects of the contract.
4. Issue written interpretations of technical requirements.
5. Monitor, document, and report Contractor's performance and notify both the CO and the
Contractor of any deficiencies.
6. Coordinate the availability of Government-Furnished Property and provides direction or assistance for site entry requests of Contractor personnel.
1.9 CONTRACTOR SPECIFIC REQUIREMENTS, ROLES, AND RESPONSIBILITIES:
1.9.1 CONTRACTOR KEY PERSONNEL: Contractor shall provide a resume of all key personnel, who will be designated in writing, to the CO with the Contractor’s technical proposal. Contractor key personnel shall be available during normal duty hours. These individuals designated as key personnel shall be able to understand, speak, read, and write the English language
A. PROJECT MANAGER(PM) : The Contractor shall provide a Project Manager (PM) who is responsible for the performance of the work and must have the following:
1. A minimum of three (3) years of documented experience of relative Solid Waste Removal and Management Service and scheduling of workflow is required.
2. The PM shall have full authority to act for the Contractor on all contract matters relating to daily operation of the contract.
3. The name and contact information of the PM shall be provided to the CO. The PM shall be available during normal duty hours. In the circumstances when the PM is not available, the Contractor shall designate an alternate who can perform the same duties during the absence.
B. QUALITY CONTROL MANAGER (QCM): The Contractor shall designate a Quality Control Manager (QCM). As a minimum, the QCM shall have at least two (2) years of documented relative Solid Waste Management services experience and shall be responsible for the overall management and implementation of the Contractor's Quality Control Plan.
C. QUALITY CONTROL INSPECTORS (QCI): The Contractor shall designate Quality Control Inspectors (QCI) personnel sufficient to monitor contract work to assure compliance with the Government’s requirements. Depending on the size of the contract, the QCM may also serve as the QCI.
1.9.2 CONTRACTOR EMPLOYEES:
A. EMPLOYEE - U.S. RESIDENCY: All CONUS Contractor employees shall be legal U.S.
residents. The Contractor shall immediately remove any employee that is not a legal U.S.
resident. The Contractor personnel shall be able to communicate clearly in the English language.
B. CONTRACTOR HIRING RESTRICTIONS: The Contractor shall not hire or subcontract with off-duty or former Government employees whose employment would result in a conflict of interest.
C. CONTRACTOR IDENTIFICATION: The Contractor shall ensure that personnel present a neat appearance and can be easily identified as a contractor employee to distinguish themselves (e.g., badge, company logo, and/or uniform) from Government employees. The Contractor shall ensure all correspondence and reports produced are marked as Contractor products or that

Contractor participation is disclosed. All Contractor owned/leased vehicles, trailers, containers, etc. shall also be clearly identified.
1.10 STANDARDS OF CONDUCT: Contractor personnel’s conduct shall not reflect discredit upon the Government. The Contractor shall ensure all personnel present a professional appearance while working on the Government installation.
A. The Contractor’s employees shall observe and comply with all local policies and procedures concerning fire, safety, environmental protection, sanitation, security, and possession of firearms or other lethal or illegal weapons or substance.
B. The Contractor shall ensure all Contractor employees, providing services under this contract, conduct themselves and perform services in a professional, safe, and responsible manner.
C. The Contractor shall remove, from the job site, any employee for reasons of misconduct or security.
D. The Contractor shall ensure employee conduct complies with 41 USC §423, Procurement Integrity.
E. The Contractor shall also ensure that no Contractor employees conduct political related activities or events on the installation.
F. The removal of such a person shall not relieve the Contractor of the requirement to provide personnel to perform adequate and timely service.
1.11 EMERGENCY MEDICAL TREATMENT SERVICES:
A. The Contractor or their employees shall immediately notify their supervisor of any incident requiring immediate emergency medical treatment.
B. The Government may provide 911 emergency support ambulance service and/or the VA medical facility, if available.
C. If injuries incurred while an employee is performing work under this contract, the contractor shall verbally notify the CO/COR no later than (NLT) two (2) hours after an emergency medical event, followed by a documented summary of events.
D. The Contractor shall be responsible for reimbursement of any cost of provided emergency medical treatment services provided to Contractor personnel.
E. Reimbursement shall be paid to the U.S. Treasury
1.12 SAFETY REQUIREMENTS:
A. In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer or his designee may determine to be reasonably necessary to safeguard and maintain all Government and Contractor property, as well as provide for the safety and well-being of personnel employed under this contract.
B. The Contractor shall comply with all local, State, and Federal safety laws, rules, and regulations.
C. All unsafe acts or conditions fostered by the Contractor or Contractor personnel may be grounds for the CO to halt all Contractor performance until such unsafe conditions are corrected.
D. The Contractor shall take due caution not to endanger personnel during performance of this contract.
PERFORMANCE WORK STATEMENT (PWS)
E. Upon discovery of a serious hazard such as, but not limited to, fire, or fuel spill, the Contractor shall immediately notify the CO and COR verbally and follow up with an email notification.
F. The Contracting Officer or his designee will notify the Contractor of any noncompliance with the contract provisions and the action to be taken.
G. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor at the work site, shall be deemed sufficient for the purpose aforesaid.
H. If the Contractor fails or refuses to comply promptly, the CO may issue an order stopping all or any part of the work and may ultimately terminate the contract for default.
1.13 SAFETY PLAN: The Contractor shall develop, maintain, and implement a written Safety Plan that will be undertaken to protect the staff of the VA Medical Center and Contractor employees, including the surrounding community for its employees.
A. The Safety Plan shall include a process detailing how the contractor will comply with the above safety requirements listed.
B. The Contractor shall submit the Safety Plan with the Contractor’s technical proposal for review and evaluation.
C. Revisions to the Safety Plan shall be submitted to the CO/COR five (5) business days prior to the effective date of change.
1.14 ACCIDENT REPORTING: The Contractor shall comply with Operational Safety and Health Administration (OSHA) for record keeping and reporting of all accidents resulting in death, injury, occupational disease, or adverse environmental impact. The Contractor shall provide a verbal report to the CO/COR within two (2) hours of each such occurrence followed up by an email documenting each such occurrence.
1.15 ENVIRONMENT AND OSHA: Any apparent conflict between compliance (with such local, State, and Federal environmental and occupational safety laws, rules, and regulations) and the requirements of the contract shall be immediately brought to the attention of the CO and COR verbally. The Contractor shall notify the CO and COR via email outlining the details for each said conflict such that a final resolution is derived. The Contractor shall be liable for all fines, penalties, and costs that result from violations of, or failure to comply with any Environmental and OSHA laws, rules, and regulations.
1.16 EMERGENCY DELAYS AND SUPPORT:
A. If the Contractor determines unsafe weather conditions or work environment is cause for an interruption of services, the Contractor shall notify the CO and/or the COR two (2) hours prior to curtailing operations.
B. The Contractor shall obtain CO and/or the COR approval to resume operations.
C. The Contractor shall coordinate with the COR in preparation for and during emergency operations situations such as inclement weather, flood, active shooter, fire or other emergency situations.
1.17 LOCAL HEALTH PROTECTION CONDITIONS: Contractor will comply with all National and VA policies with regard to health protection issues including pandemic outbreaks. Local Health Protection

Conditions (HPCON) change based on risk levels in the surrounding JAHVH area and in cooperation with local, state and national guidance. The Contractor shall comply with installation and higher headquarters policies aimed at mitigating the spread of dangerous communicable diseases.
1.18 HOURS OF OPERATION: Normal hours of operation are between the hours of 6:30 AM and 5:00 PM, Monday through Friday, excluding Federal holidays or when the Government facilities are closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Except for specials events, the Contractor shall not be required to provide services after normal duty hours, weekends, and Federal holidays. Services outside of normal duty hours must be authorized by the Contracting Officer (CO).
A. The Contractor shall coordinate with COR to establish a schedule to conduct business if services are required outside of normal hours of operation.
B. The Contractor shall maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when installations are not closed for the above reasons.
C. Specific guidance for a regulated start time does occur and is subject to change, on any given moment, depending on the circumstances. Further guidance shall be provided by the COR. As an example of additional specified service requirement timeframes, special events will require alternative work hours that will be designated by the COR and/or CO. Work hours will be part of the discussion during the kick-off meeting and as part of Government furnished data.
1.19 INSTALLATION CLOSURES: When an unforeseen installation closure occurs on a regularly scheduled day of work, the Contractor shall reschedule the work to be performed the following day unless the following day is a Saturday, Sunday, a recognized Federal holiday, and when routine work is not scheduled for that day; or reschedule the work for the next business day following the closure.
1.20 ADMINSTRATIVE INORMATION:
A. RECOGNIZED HOLIDAYS: The contractor is not required to provide service on the following list of recognized Federal Holidays:

B. TRAFFIC FLOW: The Contractor shall maintain the free flow of vehicular and pedestrian traffic while conducting Solid Waste Management Service Tasks.
C. NOISE AND DISRUPTION: The contractor is required to continuously consider the impact to the medical center in completing the required work. Any operation causing excessive noise or vibration shall be coordinated in advance with the COR. Also, movement of equipment, supplies, and waste through occupied areas, corridors, and elevators must be planned and scheduled with the COR to minimize disruption to hospital operations.
D. PARKING: It is the responsibility of contractor personnel to park only in designated parking areas. Parking information is available from the VA police and COR. The VA will not invalidate or provide reimbursement for parking violations of the contractor’s personnel under any circumstances.
E. SMOKING: Smoking is not permitted within or around VA Healthcare System facilities, except within designated areas.
F. ACCESSABILITY AND SECURITY: Contractor shall keep in mind and, to the extent applicable, follow practices that ensure the security and safety of JAH Veterans Hospital facilities, equipment, and personnel. To this end, accessibility to all work areas will be provided by the COR. No keys will be issued to Contractor employees.
G. CHANGES TO PERFORMANCE WORK STATEMENT: Any changes to this PWS shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
H. TITLE OF SOLID WASTE: The contractor shall take title of all solid waste once the waste is fully loaded into the contractor’s furnished vehicles.
I. DELIVERABLES: The Contractor shall compile historical data, prepare reports, and submit information as specified by all the deliverable requirement stated in this PWS or as directed by the CO/COR.

3.1 OVERVIEW: Solid Waste Management is a comprehensive approach to managing non-hazardous solid waste that encompasses green procurement, waste prevention, and disposal programs. The Contractor, partnering with the installation, shall determine and utilize the most cost effective Recycle and Solid Waste Management services, emphasizing innovation and commercial best practices, to maximize diversion. The Contractor shall dispose of all commercial, industrial, and community activity waste in accordance with (IAW) all applicable Federal, State, and local laws and regulations. The solid waste shall be disposed of in accordance with the Hillsborough County Solid Waste Ordinances by Contractors or Subcontractors with an authorized Hillsborough County Franchise Agreement.
3.2 DESCRIPTION OF SERVICES:
3.2.1 SCOPE: The Contractor shall determine the best and most cost-effective disposal method for solid waste management to avoid picking up an empty dumpster or container. The disposal method shall be approved by the CO and COR. The Contractor shall empty recycle/refuse containers, compacters, and roll-offs IAW PWS performance standards. In the event of demand fluctuations, frequency adjustments must be approved by CO and COR. The Contractor shall track and report to the COR all container weights and number of services per week. The Contractor shall perform services IAW approved scheduled routes for pick-up and respond to calls for service when dumpsters reach full before scheduled pick-up.
A. Hazardous Materials: The Contractor shall not handle asbestos, human remains, or compact military food heaters. The Contractor shall not handle bio-medical (also known as “red bag”) or other hazardous materials. The Contractor shall report to the COR any hazardous materials identified or discovered at the Collection Point
B. Scavenging: The Contractor shall prohibit scavenging of materials from the Collection Point.
The Contractor shall not scavenge materials.
C. Co-mingling Waste. The Contractor shall not co-mingle or transport solid waste and recyclables which shall be collected, transported, and disposed in accordance with any Hillsborough County Solid Waste Ordinance.
3.2.2 HILLSBOROUGH COUNTY SOLID WASTE ORDINANCE: The Contractor or Subcontractor must be authorized (franchised) to perform these services by the Hillsborough County Solid Waste Management Department by time of award and throughout the effective period of performance of this contract.
3.3 CONTAINERS:
3.3.1 Containers List, Location, and Site Map. The Contractor shall place all solid waste and recycle containers according to Attachment 2, “Container List, Location, Site Map”. The referenced containers are located at the JAHVH, 13000 Bruce B Downs Blvd, Tampa, FL 33612 and the Brooksville CBOC at Brooksville, FL. Attachment 2 “Container List, Location, Site Map” includes the Location, Container Type & Size, Service Type, , Tag No., Container Quantity, Waste Type, Estimated Service Quantity, Unit of Issue, and Frequency per Week/Day. JAHVH Site Map is provided for reference purposes. (CY = Cubic Yard)
PERFORMANCE WORK STATEMENT (PWS)
A. Compactor Specifications. Compactor unit shall be a 30 cubic yard self-contained unit, not less than industrial heavy gauge steel (10 gauge). All seals shall be industrial high pressure;
shall have full door seal and heavy duty ram piston. Hydraulic lines shall be ¾ inch. The compactor unit shall be equipped with continuous input air ionizing system to prevent bacterial growth and offensive odor. All installation and maintenance during the rental period shall be covered by the contractor. The contractor will need to make sure that the compactor to be provided fits and operate properly in the current enclosure (size, electrical connection, etc.)
B. Compactor Fit and Operation. Currently, there is a 30 cubic yard waste compactor unit in place located within an enclosed bay. JAHVH does not anticipate any major changes or renovations to the current enclosure if the compactor is to be replaced by a similar commercial type of container. Nevertheless, the contractor will need to ensure that the compactor to be provided fits and operates properly in the current enclosure (size, electrical connections, etc.)
C. Compactor Cleaning. In addition, this unit will require occasional washing and cleaning to remove process able waste building up and foul odor and to return the unit to a clean, sanitary, and serviceable condition. This will be coordinated through the COR via call-in requests to be completed no later than the next service day.
D. Backup 30 CY Container: Contractor shall provide, as needed, a backup 30 cubic yard container. Backup container shall be provided within 4 hours of notification, whenever needed (24 hours/day, 7 days/week). There is a 30 cubic yard compactor currently in place.
E. Emergency Telephones: A 24 hour, 7 days per week toll free number and one (1) additional cell phone number shall be provided for emergencies and repair calls. Emergency/repair calls shall be addressed and a solution provided within 3 hours of the call.
F. Laws and Ordinances: The contractor shall comply with all current county or municipal, state, and federal laws, regulations and guidelines when collecting, storing, removing, transporting, disposing of and/or recycling all wastes under the scope of this contract. Includes Hillsborough County Ordinance 96-34.
G. Container Approval. Contractor shall use provide designated containers that meet county standards and should not be overfilled or placed on public right-of-ways. All solid waste and recycle collection containers shall be CO/COR approved. Containers must be clearly marked to identify names, telephone numbers, and emergency telephone numbers.
H. Container Lids. All 8 CY containers shall be equipped with lids and locking latches where required. All solid waste container tops, lids, and doors shall be closed after emptying.
3.3.2 Maintaining Containers and Collection Area: The Contractor shall:
A. Maintain all containers in good, workable condition and ensure they must remain accessible to the customers. Check for corrosion and other damages and replace containers at the request of the COR.
B. Perform all cleaning, repair and other maintenance tasks off-VA property.
C. Return the containers to their original location after servicing, in an upright position with the lids securely in place (closed).
D. Remove all refuse on the ground within ten feet of the container, whether spilled by the contractor or placed there by VA personnel, shall be picked up by the contractor during collection. Collect debris placed at collection areas; for example, tree branches, sacks, cartons, boxes, cans, tied bundles, or other containers.

E. VA personnel regularly monitor the garbage container area and if additional containers are needed, necessary coordination to address any issues will be made at that time.
F. Container Spillage. The Contractor shall immediately pick up and clean up any spillage, debris, etc. resulting from emptying of container and compacting of solid waste, recycle material, or grease in vehicles. The Contractor shall verbally notify the CO and COR, followed by written notification if the Contractor spills or releases any hazardous substances.
G. Restoration of Real Property Damage. Any damage to Government real property by the Contractor’s operations shall be restored to the original state at no additional cost to the Government (e.g., structural damage, damage to landscape or fencing, et al).
H. Container Cleaning Schedule. The Contractor shall provide the COR with a recommended collection container cleaning schedule NLT ten (10) calendar days after contract award. The Contractor shall clean collection containers with food waste, (e.g., containers at dining facilities, cafeterias, etc.) as needed. The Contractor shall provide to the COR with a list of refuse collection containers that have received cleaning service each month throughout the Period of Performance (POP). The Contractor shall provide a Monthly Refuse Container Cleaning Schedule that shall include days of service and driver, each container location, container type, cleaning time, and cleaning method. The Contractor shall clean and sanitize all collection containers, as needed, to ensure they are clean and odorless
3.3.3 Un-serviceable Containers. The Contractor shall maintain all containers in a serviceable condition. The containers are unserviceable if any one of the following conditions exist:
A. Container does not meet specifications.
B. Container is not leak proof (storm water pollution prevention compliant).
C. Container is not enclosed (except for roll-off containers designed to be open) D. Doors, lids or hardware are missing, damaged, not operational, or do not close in the manner in which they were designed to close E. Over twenty-five percent (25%) of exposed surface is not a uniform color (painted if container is not molded or composite plastic) F. Over ten percent (10%) of exposed surface is covered with rust.
G. Container exterior is damaged (substantial dents, holes, excessive scratches, burrs, ragged edges.
3.3.4 Evaluating Containers. The Contractor shall evaluate the condition of all collection containers on a quarterly basis and provide this information in a report to the COR. The report shall include container size, serial number, location, condition rating, number of times cleaned that quarter, number of times painted that quarter, the number of times repaired that quarter, number of dumpsters moved that quarter, total number of serviceable collection containers, and number of collection containers in need of repair.
3.3.5. Container Replacements. Deteriorated solid waste containers shall be replaced or repaired by the Contractor at no cost to the Government. The Contractor shall replace containers within two
(2) working days of receiving notification. The Contractor shall furnish replacement containers when containers are removed for cleaning, painting, maintenance, or repair. The Contractor shall maintain spare containers to fulfill the replacement of containers needing to be replaced.
3.3.6 Container Condition Complaints. Complaints regarding unsanitary conditions of containers and/or area around containers, due to Contractor negligence, shall be corrected by the Contractor within four (4) hours of notification. The Contractor shall maintain the Collection Point to discourage

the harboring, feeding, and breeding of disease vectors. When responding to complaints, the Contractor shall remove all litter and waste on the ground within a ten (10) foot radius of the container.
3.4 SCHEDULES:
A. Collection schedule: The Contractor shall coordinate with the COR to develop a collection schedule that minimizes interference with Government business and submit the schedule to the CO/COR for approval. The schedule shall outline tasks to be performed at specific collection stations and identify the timeline for maintenance and cleaning tasks to be accomplished. The Contractor's schedule shall cover a 12-month period. All schedules shall be submitted to the CO/COR for approval not later than (NLT) ten (10) workdays from the award date of the contract and prior to commencing work and every subsequent option period. The Contractor shall perform all work required under the contract IAW the Contractor's approved schedules.
B. Specific Tasks Routes/Schedules Submittals. The Contractor shall submit a schedule organized in two (2) hour increments and by days of the week for vehicle routes and collection.
The report shall be submitted for CO/COR approval NLT ten (10) working days from the award date of the contract. The Contractor shall jointly monitor with the COR the fill capacities during the first thirty (30) days of performance and one (1) other time during each performance period.
C. Changes to pick-up schedule. The Contractor shall submit changes to the schedule to the CO/COR NLT ten (10) working days prior to the proposed implementation date of any change.
The Contractor shall not implement schedules or changes to the work schedule without prior written approval from the CO/COR. Proposed changes to the schedule or haul route shall be approved by the CO/COR in writing prior to route changes. The CO/COR may also provide the Contractor with requirements to make changes to the schedule as they occur.
D. Compactor. Contractor shall pick up the 30 cubic yard waste compactor, located at the receiving dock, Building #1, per the Container List and Location Schedule, Attachment 2, or as required by the facility on Monday, Wednesday and Friday between the hours of 5:00am to 7:00am local time. The average load per pull during the last year was between 4 and 6 tons.
E. Containers (30YD & 8 YD). Contractor shall pick up all 30 cubic yard and 8 cubic tard containers per the Container List and Location Schedule, Attachment 2, or as required by the facility.
F. Skipped Containers. The Contractor shall contact the COR on all containers that cannot be serviced because of blocked access, contamination, illegal material, etc. The Contractor shall contact the COR within (one) 1 hour after identifying the non-collected container. The Contractor shall provide the service date, truck or route number, driver's name, container location, time container was checked, why it could not be emptied, and comments (for notations such as a description and license number of blocking vehicle). Upon notification by the COR, the Contractor shall ensure collection of the refuse collection container NLT four (4) hours following notification.
3.5 RECYCLE SUPPORT: The Contractor shall pick up recyclables generated on site and all recyclables shall be taken for recycling to an approved and certified recycling facility.
A. Cardboard. The Contractor shall empty all cardboard recycling container(s) and all cardboard shall be taken for recycling to an approved and certified recycling facility. Cardboard recycling

services shall include the emptying of all cardboard, loading on to the Contractor owned vehicle, transportation to recycle facility. Cardboard recycling costs shall be charged to the appropriate Contract LIN which includes all costs to perform the services and that are associated with this service.
B. Bulk Items. The Contractor shall remove and dispose of bulky items (e.g., shipping cartons, scrap lumber, discarded furniture, appliances, toys, wood pallets, etc.) placed in the container at collection stations upon special request by COR.
C. Yard and Organic Waste. The Contractor shall load, transport, and deliver all yard and organic waste collected to an authorized Hillsborough County off-site landscape recycling/composting facility. Yard and organic waste includes, but not limited, to grass clippings, cut, or uprooted weeds, wood chips, cut or uprooted shrubs, leaves, logs (including extremely large logs/stumps), tree trunks, stumps, storm clean-up organics, driftwood, branches and limbs.
The Contractor shall provide pick-up of yard waste within twenty four (24) hours from notification of the COR requesting the pick-up. Payment for organic waste removal shall be invoiced based on tonnage fees and the cost to load, transport, and haul yard and organics to an appropriate recycling facility.
3.6 VEHICLES:
3.6.1 Collection Vehicle Maintenance. Collection vehicles shall be maintained and serviced IAW the manufacturer’s recommendations. The Contractor shall ensure collection vehicles receive periodic safety checks and all irregularities shall be repaired before the vehicle is operated.
A. The Contractor shall remove, all inoperable or unserviceable Contractor-furnished vehicles and equipment, off the site.
B. The Contractor shall not leave waste in collection vehicles over twenty-four (24) hour.
C. The collection vehicle operator shall immediately clean up all spills caused during the operation.
D. All operators of Contractor-furnished vehicles shall be licensed by the State and operate IAW installation traffic regulations.
E. The Contractor shall not leave vehicles unattended unless properly locked and secured.
F. Accident Reporting. In the event an accident occurs on the Department of Veterans Affairs property or involving Government personnel or property, the Contractor shall contact the VA Police immediately. A report shall be provided to the CO and COR in writing within 48 hours of any accident that shall include the following: (1) the time and date of occurrence; (2) the place of occurrence; (3) a list of personnel directly involved; and (4) a narrative or description of the accident to include chronological order of the accident and circumstances; (5) corrective action to prevent future occurrences.
3.6.2 Unserviceable Vehicles. The Contractor shall have serviceable vehicles to ensure completion of contract requirements in the event of breakdowns, repair, or other mechanical problems. Vehicles that are identified as unserviceable shall be repaired or replaced NLT three (5) calendar days at no additional cost to the Government. The Contractor shall provide maintenance and repair service.
3.7 PERMITS AND LICENSING: To operate as a commercial trash hauler in Florida, you need to obtain a business license, comply with local regulations regarding waste collection and disposal, have proper insurance coverage, and may need to acquire a specific "solid waste collection permit" depending on the county, which often requires submitting proof of financial responsibility and a waste

profile detailing the types of waste you will be handling; additionally, you must adhere to rules regarding container placement, overfilling, and proper waste segregation depending on the municipality.
A. Contractor shall submit as part of its quote copies of all applicable permits and licenses such as a waste disposal facility permits, recycling facility permits, and transportation hauling licenses.
B. As requested, the contractor shall provide proof of regulatory compliance such as inspection reports, policies and procedures, staff training records and emergency plans.
3.8 OTHER EVENTS.
3.8.1 Storms and Natural Disasters: The Contractor shall provide additional collection containers or increase frequency of services to manage the temporary demand. The Contractor shall provide additional resources as needed to facilitate returning to full operational capacity.
3.8.2 Inclement Weather Planning. Upon notification of destructive weather by the COR to prevent damage to containers, the Contractor shall collect all containers and temporarily relocate them to positions adjacent to buildings or other immovable structures. The Contractor shall provide appropriate tie-downs. The Contractor shall return all collection containers to normal positions when destructive weather conditions have diminished.
3.8.3 Inclement Weather Delay. If the Contractor determines unsafe weather conditions for continued operations, then the Contractor shall request approval from the CO/COR to suspend services. The Contractor shall suspend collection and disposal operations because of inclement weather only after receiving approval from the CO/COR. The Contractor shall resume operations with CO/COR approval.
3.8.4 Unscheduled Clean-Ups. The Contractor shall clean-up and remove solid waste on the ground at collection sites. The Contractor shall provide the appropriate containers and labor.
3.8.5 Scheduled and Unscheduled Requirements: Scheduled requirements also include recovery operations due to natural causes or man-made causes. The Contractor shall provide collection containers to adequately manage the waste volume. Unscheduled events will be coordinated through the CO/COR. Upon notification, the Contractor shall provide the required services within one (1) working day.
3.8.6 Emergency Work. The CO/COR may designate a work requirement as an emergency. The Contractor shall respond immediately to emergency work requests. The Contractor shall divert employees, if necessary, to respond to and accomplish emergency work. Emergency work would be necessary for the protection of life, health, or safety. Response time shall not exceed two (2) hours and work shall commence within four (4) hours upon notification. The Contractor shall continue working until the work is complete.
3.8.7 Installation Closures. The Contractor shall have a Project Manager, designated as emergency essential, to handle refuse collections during installation closures IAW with this PWS.
3.9 SPECIAL EVENTS:
3.9.1 Special Events Planning. The Contractor shall participate in the planning and coordinating of special events. Scheduled special events may take place installation wide.
3.9.2 Additional Services. The Contractor shall be required to provide additional collections or relocate containers for special ceremonies, security, organizational events, etc.
A. Relocations shall be accomplished, with the approval from the CO and COR, not more than eight (8) working hours after the verbal request by phone.

B. Major events requiring solid waste removal and recycling support will be planned two (2) weeks in advance and the Contractor shall provide services necessary to support the event.
C. The Contractor shall place solid waste and recycling collection containers at the event or locations for the period specified by the COR.
D. These events shall be at no additional cost to the Government if accomplished during normal duty hours and days.
E. If these events occur on holidays, weekends or after normal duty hours and days, then the Contractor shall charge to the special event to the applicable Contract LIN.
F. The Contractor shall provide transportation, placement, and removal of additional special event collection containers on an on-call basis when directed by the CO/COR.
G. The COR will receive all special collection requests from the various hospital customers /requesters and will notify the Contractor prior to any special collection container support placements.
H. The COR will identify each Organization and their request for services and will contact the Contractor with placement and removal dates/times, placement location, types of materials that will be disposed of by the requesting Organizations and any additional information pertaining to the service.
I. Extra collection container costs will include placement, transportation, and final removal per each request. Empty and returns of any collection container will be an additional charge and charged to the appropriate Contract LIN for empty and return services.
J. Any additional/extra collection containers placed at any location shall be included on the common level of servicing route for all collection containers serviced during normal business hours and days, unless otherwise specified by the COR/CO.
K. All collection containers shall be emptied as often as needed during normal business days and hours, at no additional cost within the common of level of service described in this section.
L. Emptying of any additional special event collection containers will also be paid based on tonnage as included under the common level of services and at no additional cost to the Government.
M. If the Contractor chooses to or is directed to provide any additional dumpsters or roll-offs to support any common level of trash or recycle services due to overabundance of trash or recycling, then this will be at no additional cost to the Government for the additional container and payment will be based on the common level of services as reflected on the Contract LIN addressing the respective common level of service charge.
N. If the Contractor is unable to keep up with any common level of trash or recycle services and needs to or choses to use additional containers or vehicle support, this will be at no additional cost to the Government for the additional container/s and/or vehicles.
O. Government and Contractor discussions can occur for each event if there is a need to come to an agreement on the level of services requested for each event.
P. Any collection container defined as reimbursable will be coordinated and tracked by the COR
3.10 PROCEDURES and REPORTS.
A. Procedure Manual. Contractor shall provide a fully detailed written description and procedural manual of the methodology that will be used to collect, transport, and dispose of the solid waste

and recyclables. Contractor shall provide a Procedure Manual with the Contractors Technical Proposal.
B. Work Control, Records, and Reports Plan (WCRRP). The Contractor shall develop and implement a WCRRP to include all information control procedures necessary to ensure timely and acceptable accomplishment of all contract data requirements, and permit tracking of work in progress. The Contractor shall submit a WCRRP to the CO/COR for approval NLT ten (10) days after contract performance start date, and modifications to the plan NLT seven (7) working days prior to implementation of any changes. The Contractor shall revise, modify, and resubmit the plan for approval annually. The Contractor shall accurately, completely, and legibly prepare and submit the documentation and automated reports to the CO/COR. Reports shall include any supplemental charts, photographs, manufacturer's pamphlets, etc., necessary to complete the report.
C. Monthly Summary Report. The Contractor shall provide a monthly summary report of tonnage landfilled as solid waste and recycled. All solid waste shall be paid per ton, unless otherwise specified. All tonnage fees must be accommodated with the waste facilities weight ticket.
D. Container Condition Report. The Contractor shall evaluate the condition of the collection containers monthly and provide this information in a quarterly report to the CO/COR.
E. Regulatory Agency Inspections. The Contractor shall submit a written report of any inspection visit by representative(s) of any regulatory agency and immediately notify the CO.
This report shall include the representatives' names, titles, identification numbers, and agencies, reason(s) for the visit, and comments or findings.
