About this file

This is a Contractor Performance Data Sheet template that offerors must complete for the Marine Corps solicitation M67004-25-R-0002 for Stevedoring and Routine Terminal Services at Blount Island Command. The form requires contractors to provide details about previous contracts of similar scope and complexity, including contract numbers, types, descriptions, locations, periods of performance, and customer points of contact.

The template includes 12 key evaluation areas across four categories: Quality (addressing similarity of work, terminations, non-conformance issues, and quality awards), Customer Satisfaction, Timeliness (focusing on schedule adherence and liquidated damages), and Business Relations (covering subcontractor management and working relationships with government personnel). Contractors must provide explanatory narratives for certain responses and describe any corrective actions taken for past performance issues. The completed forms will be used alongside Past Performance Questionnaires, PPIRS, and CPARS data to evaluate offerors' past performance and inform the best value award decision.
