About this file

This is a Vendor ACH Authorization Form used by Brookhaven National Laboratory/Brookhaven Science Associates (BNL/BSA), which operates under contract with the U.S. Department of Energy. The form is a standard banking document template that allows vendors to set up direct deposit payments through the Automated Clearing House (ACH) system.

The form requires vendors to provide their company information (including name/DBA, vendor number, and Tax ID), banking details (financial institution, routing number, and checking account number), and authorization signatures. The authorization clause grants BNL/BSA permission to initiate ACH credit entries and corrections to the specified account, with the agreement remaining in effect until written notification of termination or modification is received, subject to NACHA regulations.
