About this file

This is an Appendix G competency assessment document that establishes performance evaluation criteria for pharmacy staff conducting daily cleaning and disinfection activities in controlled environments.

The assessment is divided into three sections evaluating different cleaning scenarios. The Non-Hazardous Daily Cleaning and Disinfection section requires staff to demonstrate competency in identifying TB1-3300 and Peridox as appropriate germicidal agents, selecting proper cleaning equipment, following USP 797 hand hygiene and garbing protocols, applying lint-free wipes saturated with TB1-3300 with a 1-minute dwell time, cleaning all pass-throughs and work surfaces, mopping floors using designated equipment starting opposite the entry door, moving carts and shelving to ensure complete floor coverage including wheel cleaning, and documenting actions on Daily Cleaning Logs. The Hazardous Daily Cleaning and Disinfection section mirrors these requirements but specifies Peridox as the sole germicidal agent with an extended 3-minute dwell time and includes the requirement that areas dry completely before compounding begins. Both sections require completion of residue removal using sterile 70% isopropyl alcohol. The Monthly section addresses sporicidal cleaning of buffer and ante-areas, including ceiling and wall cleaning, cart disinfection using specialized mop handles, and thorough cleaning of vertical and horizontal surfaces. All sections require documentation on appropriate cleaning logs and include rating scales for evaluator assessment with space for comments. Qualified pharmacy staff must conduct all observations and evaluations according to USP 797/800 standards and facility policy.

Appendix G

Competency Assessment: Daily Cleaning and Disinfection

Employee: _______________________________ Position:

Evaluator: ________________________________ Initials: _____ Observations and subsequent evaluation must be made by qualified pharmacy staff. Using the space designated next to each criteria, the Evaluator will note the following:

NON-HAZARDOUS DAILY CLEANING AND DISINFECTION

Skill Initials Comments

Identifies TB1-3300 and Peridox as the appropriate germicidal cleaner/disinfectants for non-hazardous cleanroom use and where they are located.

Identifies appropriate cleaning equipment for cleanroom use and where they are located. Uses appropriate mop/wipe for the type of surface to be cleaned (floors, bins, etc.)

Follows hand hygiene and garbing process per USP 797 and pharmacy policy.

Uses a lint free wipe saturated with TB1-3300 (dwell time of 1 minute is observed) and allows area to dry completely. Use sterile 70% IPA for removal of residue.

Cleans all pass throughs, counters, sinks, and easily cleanable work surfaces (i.e., the top of carts, top of stools).

Mops floors using designated floor mop and Peridox starting on the wall opposite the room entry door using even strokes toward the operator.

Moves carts and rolling shelving to ensure that the entire floor surface is cleaned. Ensure wheels are cleaned.

Documents actions on the Daily Cleaning Log for Controlled Environments.
