About this file
This is an appendix containing a competency assessment form for evaluating employee proficiency in proper cleaning and disinfection procedures of Laminar Airflow Workstations (LAFWs).
The assessment form is designed to be completed by qualified pharmacy staff and documents evaluation of specific cleaning criteria through an initials-based sign-off system. The evaluation covers daily and monthly cleaning protocols, including the use of TB1-3300 followed by sterile alcohol for daily cleaning, and Peridox followed by sterile alcohol for monthly cleaning, with appropriate dwell times observed between agents. The form specifies detailed procedural requirements for cleaning hood components in a prescribed sequence: ceiling first using horizontal strokes from back to front, back wall second with horizontal overlapping strokes, sides third using vertical overlapping motions, and deck last with horizontal overlapping strokes. Additional monthly procedures include lifting the diffuser screen to clean its back and bottom surfaces and the area beneath the work surface. The assessment also requires cleaning of deck items such as repeater pumps using a top-to-bottom approach including power cords. Behavioral criteria are evaluated to ensure employees do not contaminate previously cleaned surfaces, place items horizontally to maintain airflow, or introduce contamination through coughing, sneezing, gum chewing, or excessive talking. The form includes fields for employee name, evaluator name and initials, date, and additional comments section for documentation purposes.
