About this file

This is a competency assessment form designed to evaluate pharmacy staff on proper cleaning and disinfection procedures for Biological Safety Cabinets (BSCs).

The form documents observations of employee performance across eleven specific criteria that must be evaluated by qualified pharmacy staff. Daily and monthly cleaning procedures include using Peridox followed by sterile alcohol on the laminar airflow work surface (LAFW) with appropriate dwell times between agents, cleaning the easy reach tool with sterile 70% isopropyl alcohol wipe, and systematically cleaning hood surfaces in a prescribed sequence: ceiling first using horizontal back-to-front strokes, back wall second with horizontal side-to-side overlapping strokes, sides third with vertical back-to-front overlapping strokes, and deck last with horizontal side-to-side overlapping strokes. The IV bar and hooks are cleaned by hand. Monthly cleaning includes an additional requirement to clean the area beneath the work surface. Additional evaluation criteria address proper handling of deck items such as repeater pumps, prevention of contamination to previously cleaned surfaces, horizontal placement of items to prevent airflow blockage, and employee personal conduct to avoid hood contamination through coughing, sneezing, gum chewing, or excessive talking. The evaluator initials each criterion in the designated space to signify satisfaction, with a comments column for additional notes and an additional comments section at the form's conclusion.
