About this file
This is a Customer Satisfaction/Quality of Service evaluation form designed for use by government evaluators to assess contractor past performance on federal contracts.
The form contains thirteen quantitative performance questions and one qualitative section, using a five-point rating scale (5=Outstanding, 4=Excellent, 3=Acceptable, 2=Marginal, 1=Unacceptable, 0=Unknown). The evaluation criteria assess overall contractor performance, conformance to contract requirements, working relationship quality, responsiveness to concerns and corrective actions, flexibility in responding to changing needs, problem resolution effectiveness, invoice accuracy and timeliness, schedule adherence, and adequacy of personnel, supplies, and equipment. Additionally, the form includes yes/no questions regarding performance awards or incentives, invoice payment processing, and whether cure notices, show cause letters, payment suspensions, or terminations were issued. A final yes/no question asks whether the evaluator would conduct business with the contractor again. The form concludes with Section IV, which provides space for evaluators to submit additional narrative comments regarding the contractor's past performance. This instrument serves as a standardized tool for documenting contractor performance history that can inform future contract award decisions.
