About this file

This document is a Small Business Subcontracting Certification form for solicitation 36C25725Q0315, related to a VA surgical equipment procurement. The certification provides two options for vendors: (1) submit a detailed subcontracting plan that addresses engagement with various small business categories (veteran-owned, service-disabled veteran-owned, HUBZone, small disadvantaged, and women-owned businesses), or (2) certify that the offeror has sufficient in-house capability to perform the contract work without subcontracting. The form is specifically linked to the VA North Texas, Dallas facility's project for operating room surgical lights, anesthesia booms, equipment booms, and video integration solutions across 12 operating rooms.
