About this file
This is a Summary of Benefits and Coverage (SBC) document for the City of Edinburg's PPO Active Plan provided by Blue Cross and Blue Shield of Texas for the coverage period October 1, 2023 through September 30, 2024. The document outlines the cost-sharing structure and covered services for individual and family health insurance plans under a preferred provider organization (PPO) model. Key coverage details include in-network deductibles of $2,000 individual/$4,000 family and out-of-network deductibles of $4,000 individual/$8,000 family, with out-of-pocket limits of $6,500 individual/$13,000 family for in-network services and $19,500 individual/$39,000 family for out-of-network services. The plan covers primary care visits at $35 per visit, specialist visits at $50 per visit, emergency room care at $350 per visit plus 20 percent coinsurance, and inpatient hospital stays at 20 percent coinsurance for in-network providers. Preventive care, child immunizations through age six, diagnostic tests, and certain prescription drugs are covered before the deductible is met. The document provides specific cost-sharing amounts for imaging services, prescription medications across multiple tiers, outpatient and inpatient surgeries, mental health services, maternity care, rehabilitation services, skilled nursing care, and hospice services, with all copayments and coinsurance amounts applied after deductibles are satisfied for in-network providers.
The plan explicitly excludes coverage for acupuncture, bariatric surgery, cosmetic surgery, adult dental care, infertility treatment, long-term care, non-emergency care outside the United States, private-duty nursing, routine foot care, and weight loss programs. Additional covered services include chiropractic care, hearing aids limited to one per ear per 36-month period, and routine adult eye care, though limitations apply. Out-of-network providers typically require members to pay 30 to 50 percent coinsurance after applicable deductibles, with the exception of emergency room care, which carries a flat $350 copayment plus 20 percent coinsurance regardless of network status. The plan does not apply plan deductibles to emergency room services, inpatient hospital expenses, skilled nursing care, home health care, hospice services, and certain preventive services, though a $250 per-admission deductible applies to out-of-network inpatient hospital stays. The document includes three coverage examples demonstrating member cost-sharing under different healthcare scenarios: a pregnancy and delivery totaling $4,010 out-of-pocket costs, routine diabetes management totaling $1,620 out-of-pocket costs, and emergency fracture care totaling $1,980 out-of-pocket costs, all based on in-network provider utilization.

Summary of Benefits and Coverage: What this Plan Covers & What You Pay for Covered Services Coverage Period: 10/01/2023 – 09/30/2024
City of Edinburg: PPO Active Plan Coverage for: Individual / Family | Plan Type: PPO
Blue Cross and Blue Shield of Texas, a Division of Health Care Service Corporation, a Mutual Legal Reserve Company, an Independent Licensee of the Blue Cross and Blue Shield Association
The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately.
This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, call 1-800-521-2227 or at
For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at www.healthcare.gov/sbc-glossary/ or call 1-855-756-4448 to request a copy.
Important Questions Answers Why This Matters:
What is the overall deductible?
For In-Network: $2,000 Individual / $4,000 Family For Out-of-Network: $4,000 Individual / $8,000 Family
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Are there services covered before you meet your deductible?
Yes. Services that charge a copayment, prescription drugs, emergency room services, inpatient hospital expenses, certain preventive care, and In-Network diagnostic tests, home health, skilled nursing, and hospice are covered before you meet your deductible.
This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at www.healthcare.gov/coverage/preventive-care-benefits/.
Are there other deductibles for specific services?
Yes. Per occurrence: $250 Out-of-Network inpatient admission. There are no other specific deductibles.
You must pay all of the costs for these services up to the specific deductible amount before this plan begins to pay for these services.
What is the out-of-pocket limit for this plan?
For In-Network: $6,500 Individual / $13,000 Family For Out-of-Network: $19,500 Individual / $39,000 Family
The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met.
What is not included in the out-of-pocket limit?
Premiums, balance-billing charges, and health care this plan doesn’t cover.
Even though you pay these expenses, they don’t count toward the out-of-pocket limit.
Will you pay less if you use a network provider?
Yes. See www.bcbstx.com or call 1-800-810-2583 for a list of network providers.
This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing).
Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Do you need a referral to see a specialist? No. You can see the specialist you choose without a referral.
