About this file

This document is a Procurement File Memorandum from the Arizona Department of Health Services, specifically from the Planning & Operations division, located in Phoenix, Arizona. The memorandum involves Contract CTR050596 for the BWCH Health Start Program in Mohave County, with the original contract (CTR050592) awarded on July 6, 2021 through Request for Proposal BPM002372.

The contract is for the Health Start Program, with an initial one-year term and options to renew annually, not to exceed five years total. The procurement file indicates that Amendment Three was processed to extend the contract, which was signed on July 17, 2023, and executed on July 24, 2023, slightly after the original expiration date of July 6, 2023. Despite the slight delay in execution, the state determined it was in the best interest to reinstate the contract for the remaining term, as the services continue to be needed. The document is signed by Lucas Terry (Procurement Specialist) and approved by Gina Corwin (Chief Procurement Officer).
