About this file

This document is a Contract Amendment issued by the Arizona Department of Health Services (ADHS), Office of Procurement, for the Health Start Program. The contract (CTR050596) involves Mohave County Health Department and was originally awarded on July 6, 2021, through Request for Proposal BPM002372.

The amendment extends the contract for the fourth (4th) year through July 5, 2024, which aligns with the original contract's provisions allowing annual renewals not to exceed five (5) years. The contract was signed by the contractor on July 17, 2023, and executed by the procurement officer on July 24, 2023, slightly after the original expiration date. Despite the slight delay in execution, the procurement file memorandum indicates that the services are still needed, and it is in the best interest of the state to reinstate the contract for the remaining term.
