CONTRACT: CS269001

Laundry Services Cost sheet (Page 1 of 3)

In compliance with your Invitation for Bids for laundry services, I hereby propose to perform the work, as specified during the term July 01,2026 through June 30, 2028, at the Contractor’s specified rates specified on Page 13.

Complete pages 12, 13 and 14 and enter your total bid amount on page 13. A flat per-pound rate for each type of laundry processing must be quoted to be considered for award. Include all three (3) pages of the cost sheet along with any other documents required to be submitted with your bid.

The bid will be evaluated based on comparison of rates provided for the Standard Laundry Services, Healthcare Laundry Services and Delivery Charges. Rates quoted for bundling, sorting, counting and wrapping are required but will only be factored in the evaluation if needed to break a tie.

Total hours bid for 1-year (On-site Supervisory, Management and Labor): _________ hours.

*Excludes health, dental and vision benefits as specified in Exhibit B, Item #8.

Bidder understands that the approximate pounds of laundry per month listed on page four (4) under Bidder Minimum Qualifications is an estimate only. There may be times when items not identified/listed in the agreement may need to be laundered; in such cases, the Contractor shall contact Canyon Springs’ Contract Manager to negotiate a cost for cleaning such an item.

The Contractors price per trip (delivery/pickup) to Canyon Springs, which should address the actual costs of labor and transportation. Bidder may be reimbursed for additional transportation costs exceeding the fuel benchmark price identified by the Contractor per a fuel surcharge monthly adjustment based on a rounded average of the national U.S. on-highway average price for a gallon of diesel fuel in California as published by the Energy Information Administration at:

https://www.eia.gov/petroleum/gasdiesel/ .

A fuel surcharge is designed to reimburse the Contractor for increased fuel costs experienced in the process of transporting items to/from Canyon Springs and is not designed to be a profit center. The fuel surcharge calculation to be used is:

The average cost for fuel for the prior month (4-5 weeks) as determined by the U.S. Energy Information Administration (EIA), minus (-) Contractor’s identified “Fuel Surcharge Benchmark Price,” divided (/) by the average miles per gallon equals the fuel surcharge rate, multiplied by (x) the Miles Driven Round Trip, multiplied by (x) the number of trips made in the prior month to/from Canyon Springs provides the fuel surcharge amount that can be charged/invoiced.

Example:

Contractor’s benchmark price = $2.501 per gallon of diesel fuel Contractor’s miles driven per trip = 200 miles Contractor’s average miles per gallon (MPG) = 6 EIA monthly average for diesel fuel in the month of April 2018 = $2.694 per gallon

$2.694 (EIA average)- $2.501 (Contractor’s benchmark) = $0.193/6 (MPG) = $0.032 (surcharge rate) x 1,600 miles (200 miles per trip x 8 trips) = $51.36 fuel surcharge amount for the month of April 2018

If the EIA’s on-highway average price for the prior month is less than the benchmark price identified by the Contractor, Canyon Springs will not authorize payment of a fuel surcharge adjustment for the month.

Contractor’s monthly invoice shall specify any fuel surcharges for the prior month, using the fuel surcharge benchmark price, the average miles per gallon, and miles driven information originally identified by the Contractor in the bid for this contract.
