About this file

This file is a Vehicle Equipment Adaptation Prescription and Agreement (VEAPA) workbook and bid form from the Maryland State Department of Education's Division of Rehabilitation Services (DORS), specifically contract number CS# 25-07. The document details a comprehensive vehicle modification project for a consumer with a spinal cord injury (C5/C6), involving adaptive equipment for a 2024 Chrysler Pacifica Touring L. The modification includes specialized equipment such as a Braun XT conversion with a fold-out ramp, power 6-way transfer seat, hand controls, secondary controls, wheelchair tie-downs, and customized driving adaptations to support the consumer's independence and employment access.

The procurement requires bidders to be NMEDA QAP/MQAP certified and includes specific communication protocols with detailed milestone reporting requirements. Invoicing can be done through a specific online portal, with partial invoices allowed for equipment over $5,000, and final invoicing upon vehicle delivery. The bid includes one year of equipment maintenance, and vendors must submit various documentation including warranty documents, consumer signed forms, and equipment disclosure forms. The total project involves precise specifications for wheelchair accommodation, driving controls, and vehicle conversion, with an emphasis on meeting federal and state vehicle safety regulations.

DORS CS# 25-07
VEAPA WORKBOOK AND BID FORM INSTRUCTIONS
This Excel Workbook contains a list of prescribed vehicle equipment as well as the Bid and Signature Form. The different worksheets can be accessed by clicking the different Tabs at the bottom of this Workbook.
The Bidder is to provide pricing for "Equipment Cost" and "Labor Cost" in the appropriate cells to the right of each piece of prescribed equipment. Those are the only numbers that are required by the Bidder.
Bidders should not calculate any Total Costs.
Excel automatically adds and totals each section and then populates those calculations and totals to the last form in the Workbook; which is the "Bid Form & Signatures" Tab.
Bidders should not "un-protect" the Workbook or Worksheets within the Workbook.
Do not alter the Bid Form or the Bid Form may be rejected.
The Bid Form is to be signed and dated, where requested, by an individual who is authorized to bind the Bidder to the prices entered on the Bid form.
Bidders must be NMEDA QAP/MQAP certified to be awarded a vehicle modification contract with DORS.
If Bidder has difficulty with this Excel Workbook contact Pam Post at PamJ.Post@maryland.gov for assistance with the Workbook.
If Bidder has questions or needs clarification on specifications prescribed in this VEAPA please email them to adrain.pulliam-smith@maryland.gov and copy brittany.owens1@maryland.gov at Maryland State Department of Education (MSDE) procurement. All inquiries must be made in writing via email.
