About this file
This is a Cost Proposal Template provided by the City of Long Beach, Department of Health & Human Services, Office of Youth Development, for the Futures First Program Request for Proposals in California. The template is designed to guide qualified service providers in preparing their budget submissions for delivering certified life coaching services and individualized case management to opportunity youth. The document requires applicants to submit a separate cost proposal for each project using the provided Excel file format, with detailed sections for personnel expenses, non-personnel/other expenses, and indirect/overhead costs. Personnel costs should include salaries and wages for employees working directly on the project, designated as full-time or part-time, along with fringe benefits. Non-personnel/other expenses encompass direct operating costs such as meeting expenses, travel, program supplies, postage, printing, and office rent allocations, as well as any consultants or sub-grants to partner organizations.
The budget template allows for up to 10 percent of indirect costs, which cover essential organizational expenses such as personnel administration, staff development, risk management, general liability insurance, tax and regulatory compliance, utilities, and maintenance. The template includes yellow highlighted cells with automatic calculation formulas to compute costs and subtotals. Applicants must provide a cost narrative explaining their proposed budget, include back-up documentation for all personnel costs, and submit completed budget information including the applicant name, budget contact name and phone number, project name, and project dates. The template provides a clear framework for calculating total expenses by combining personnel costs, non-personnel/other expenses, and the allowable indirect/overhead costs.
