About this file
This is a Contract Intake Form required for potential consultants and entities seeking contracts, agreements, or memorandums of understanding with the Office of Superintendent of Public Instruction (OSPI), State Board of Education (SBE), Professional Educator Standards Board (PESB), or Financial Education Public-Private Partnership (FEPPP) in Washington state. The form is specifically related to a water safety pilot program that aims to develop and implement curriculum in 50 public schools during the 2024-25 school year.
The form requires detailed business information including legal name, DBA name, taxpayer identification numbers, statewide vendor numbers, Washington state business license status, and certification details for woman-, minority-, or veteran-owned businesses. It also requires disclosure of any state employment connections and subcontractor usage. The form emphasizes the requirement for contractors to register as a Statewide Vendor for payment processing and mandates the use of the state's Access Equity system for payment tracking when subcontractors are involved.
