About this file

This is a Consultant Reference Check instruction document and evaluation form for the City of Phoenix Office of the City Engineer's Design and Construction Procurement section related to the Cactus Park Precinct General Obligation Bond project (OPENGOV ID NO.: PD00000077). The document provides detailed instructions for consulting firms responding to the Request for Qualifications to submit up to three performance evaluation references from agencies or entities for which they have recently provided similar services. References must be submitted by email to soq.referencechecks@phoenix.gov by 12:00 pm Phoenix time on March 20, 2026, with submissions directed to Julie B. Smith, Contracts Specialist II. The document includes a sample cover letter template for firms to use when requesting reference evaluations from past clients and comprehensive instructions for reference providers completing the Consultant Performance Evaluation Form.

The reference check process evaluates consultant performance across seven rating categories: business relationship quality, team consistency with proposal, proactive problem-solving, on-time and on-budget completion, quality of work, recommendations for efficiencies or innovative ideas, and willingness to contract again. Each reference is scored on a four-point scale (Unsatisfactory at 0.25 points, Poor at 0.50 points, Good at 0.75 points, and Excellent at 1.0 point), with a maximum of 7 points per reference for a total of 21 points available in the overall evaluation process. The completed evaluation forms become public record and may be released to the contractor or other entities upon request. Failure to submit reference evaluations by the deadline negatively impacts the firm's proposal evaluation.
