About this file
This document is a consultant reference check form and instructions issued by the City of Phoenix Office of the City Engineer's Design and Construction Procurement section for the Cactus Park Precinct - General Obligation Bond project (OPENGOV ID: PD00000077). Responding consultants must submit up to three reference checks from agencies or entities for which they have recently provided similar architectural, engineering design, construction administration, and inspection services. References must be submitted by email to soq.referencechecks@phoenix.gov by 12:00 pm Phoenix time on March 20, 2026, with all forms directed to Julie B. Smith, Contracts Specialist II, at julie.b.smith@phoenix.gov or 602-534-2418. Each reference evaluation is worth up to 7 points, with a maximum of 21 points available in the overall evaluation process. The Consultant Performance Evaluation Form must be completed by the agency owner or owner's representative directly responsible for project oversight and must address seven rating criteria: business relationship quality, team consistency between proposal and execution, proactive problem-solving, on-time and on-budget completion, quality of work, recommendations for efficiencies or innovative ideas, and willingness to contract again.
The reference evaluation form uses a four-point rating scale where 1 = Unsatisfactory (0.25 points), 2 = Poor (0.50 points), 3 = Good (0.75 points), and 4 = Excellent (1.0 point). All rating areas must be scored, and comments are optional but appreciated. The completed reference forms become public record and may be released to the contractor or other entities upon request. The document includes a sample cover letter template that firms may use when requesting references from previous clients and specifies that failure to submit reference checks by the deadline will negatively impact the proposal.
