Consolidated State Law Affidavit Summary
This is a Consolidated State Law Affidavit for the City of Gainesville, Florida, establishing compliance requirements for vendors bidding on the Regional Transit System's Bus Shelters and Amenities procurement. The affidavit mandates that all bidding entities attest to compliance with multiple Florida statutes covering public entity crimes, scrutinized company restrictions, E-Verify requirements, human trafficking prohibitions, and foreign country of concern restrictions. Bidders must certify that neither they nor their affiliates appear on the convicted vendor list, discriminatory vendor list, or antitrust vendor list; are not engaged in boycotts of Israel or operations in sanctioned countries; and maintain current registration and active usage of the federal E-Verify system for all new employees. Additionally, entities must confirm they do not utilize coerced labor and are not owned by or controlled by governments of foreign countries of concern including China, Russia, Iran, North Korea, Cuba, Venezuela, or Syria.
The affidavit requires sworn certification under penalty of perjury that all statements are true and accurate, with execution authorized by a designated representative of the bidding entity. The document must be notarized by a Florida notary public, with the affiant providing their signature, printed name, title, entity name, and date of execution. All subcontractors, if applicable, must provide similar certifications of compliance with E-Verify requirements and affidavits confirming they do not employ unauthorized aliens. Non-compliance or falsification of this affidavit may result in contract termination and legal consequences, making it a critical prerequisite document for participation in the City of Gainesville procurement process.

If there is a good faith belief that one of Entity’s subcontractor(s) has knowingly violated the Section 448.09(1), Florida Statutes, but the Entity has otherwise complied with its obligations thereunder, then the Entity will be required to immediately terminate the contract with the subcontractor in order to continue providing services to the City of Gainesville.
