About this file

This is a Federal Tax Liability and Recent Felony Conviction certification form required for the City of Gainesville's Regional Transit System (RTS) Bus Shelters and Amenities project in Florida. The form must be completed, signed, and uploaded with all bid submittals, with failure to do so resulting in automatic disqualification. The contractor must certify that it does not have any unpaid Federal tax liability that has been assessed and exhausted of all judicial and administrative remedies, and that it has not been convicted of any felony criminal violation under Federal law within the preceding 24 months.

The contractor is required to flow this certification requirement down to all subcontractors and lower-tier participants regardless of subcontract value. The form includes blank spaces for the contractor's company name, authorized representative title, signature, and date of certification. This standard compliance document is a prerequisite for bid consideration on the project and ensures that bidders meet basic Federal eligibility requirements before contract award.
