About this file
This is a Common Carrier Attestation form required by Pinellas County, Florida for contractors bidding on fire protection system inspections and repair services. The attestation, mandated under Florida Statute 908.111 and 92.525, requires contractors to confirm compliance with state law regarding the transportation of persons and authorization status. The form must be completed and signed by an authorized representative of the contractor under penalties of perjury, with the contractor providing their Federal Work Authorization User Identification Number and the specific Pinellas County contract name and number. Bids for the fire protection system services are due July 7, 2026, and the scope includes testing, maintenance, inspection, certification, and repair of fire system components including fire pumps, valves, backflow preventers, piping, sprinkler heads, standpipes, chemical suppression systems, detection devices, alarm systems, fire panels, and deluge valves at various County locations.
Pinellas County reserves the right to terminate the associated agreement for cause if the contractor is found in violation of the Common Carrier Attestation. This attestation is one of three required compliance documents, alongside a Foreign Countries of Concern Affidavit and a Human Trafficking Affidavit. No contract value, term length, incumbent contractor information, or specific pricing details are contained within the attestation form itself. Contractors must reference the Fire Protection System Inspections and Repair ITB document (posted June 11, 2026), the Qualification Submittal Form (posted June 11, 2026), and the Pricing and Location Sheet (posted June 10, 2026) for complete procurement details, technical specifications, service locations, and evaluation criteria.
