About this file

This is a Request for Quote (RFQ) for NIOSH Medical Services issued by the Centers for Disease Control and Prevention (CDC). The solicitation number is RFQ 75D301-26-Q-78995, and it is a combined synopsis/solicitation for commercial items using FAR Subpart 13.1 Simplified Acquisition Procedures. The acquisition is set aside for total small business under NAICS code 621999 (All Other Miscellaneous Ambulatory Health Care Services) with an SBA size standard of $21 million annual revenue. CDC intends to award a firm-fixed-price contract to the lowest price technically acceptable offeror whose offer provides the best value to the Government.

Offerors must submit pricing proposals for estimated annual medical testing services, including line items for test pricing (CLINs 0001, 1001, 2001, 3001, 4001) and travel costs (CLINs 0002, 1002, 2002, 3002, 4002) to perform the estimated tests outlined in the Statement of Work. Quotes should include total price for all CLINs and capabilities demonstrating ability to perform required medical testing at designated geographic locations. CDC will assess technical acceptability based on proper tests, pricing, and stated capacity to meet geographical requirements, then award to the lowest-priced technically acceptable proposal. The evaluation will assess price for fair and reasonableness, looking for discounts on open-market pricing. Proposals must be submitted via email to wlv7@cdc.gov by Tuesday, June 2, 2026, no later than 3:00 PM Eastern Time. Questions must be submitted by Thursday, May 28, 2026, at 3:00 PM ET. Offerors must be registered in the System for Award Management at time of quote submission and must include FAR 52.212-3 representations and certifications.
