This Task Assignment authorizes Wood Environment & Infrastructure Solutions, Inc.

to conduct annual groundwater sampling at the above referenced site. All work will be conducted in accordance with the scope of work and cost proposal submitted on June 9, 2022. Specific subtasks are detailed below:

Subtask 1: Project Management

Project management is calculated at 13% of the direct labor costs.

Subtask 2: Annual Groundwater Sampling

This subtask includes labor, laboratory, subcontractor and equipment costs to perform annual groundwater sampling. Personnel will collect groundwater from twelve (12) microwells to include MW-1, MW-2I, MW-3, MW-4, MW-5, MW-6, MW-8, MW-10I, MW-14R, MW-16, MW-18I and DW-1. Each of the samples will be analyzed for chlorinated volatile organic compounds (CVOCs) using EPA method 8260. The groundwater sample collected from source area micro-well MW-1 will be analyzed for total recoverable petroleum hydrocarbons (TRPHs) using the FL-PRO method to monitor for potential groundwater impacts from the EOS-PRO amendment that was placed in the source removal open excavation. MW-1, MW-2I, MW-3, MW-5 and MW-13 will be sampled for natural attenuation parameters including total iron, ferrous iron, sulfate, sulfide, nitrate, nitrite, total organic carbon, along with dissolved gases methane, ethene, and ethane using EPA method RSK175. Subcontractor costs included the proper disposal of the purge water that is containerized in a 55-gallon drum. The deliverable for this subtask will include an Annual Groundwater Monitoring Report detailing the activities and findings from the sampling event, including field notes, lab reports, IDW disposal documentation, updated tables and figures to include plume map and groundwater flow direction figure.

Task Assignment Negotiation Summary HW688 Task Assignment DC005A

Summary Subcontractors for purge water disposal were chosen on a lowest-cost basis. Documentation of the costs is included in the attached CEF, Level of Effort worksheets, and the contractor’s proposal. The final deliverable due date is November 4, 2022 and the completion date for the task assignment is January 4, 2023. The total cost for Task Assignment DC005A is $10,579.07 www.floridadep.gov

RE: Cost Proposal to Conduct Supplemental Site Assessment Baymeadows Place Cleaners 9551 Baymeadows Road Jacksonville, Duval County, Florida FDEP FAC ID No: 16/9502088

ERIC_4558

Task Assignment No: HW668-DC005A Wood Project No: 67832270XX

Dear Ms. Shirah, Wood Environment & Infrastructure Solutions, Inc., (Wood) is pleased to provide this cost proposal to conduct the natural attenuation monitoring (NAM) groundwater sampling events at the Baymeadows Place Cleaners site located in Jacksonville, Florida per the approval letter dated December 7, 2021. All sampling activities conducted during this groundwater sampling event will be in accordance with the Standard Operating Procedures (SOPs) described in the original Site Assessment Workplan dated October 3, 2014. The last sampling event at the site took place on September 2, 2021. Figure 4 shows the locations of the existing micro-wells to be sampled.

The proposed scope of work is detailed below:

Subtask 1 – Project Management This subtask includes all tasks associated with managing and scheduling the field events at the site and submitting the deliverables.

Sub Task 2 - Semi-Annual Groundwater Sampling Event

Wood will mobilize a two-person crew to the site to conduct the groundwater sampling activities.

Prior to collecting groundwater samples, all 20 micro-wells will be opened and allowed to equilibrate and then water level measurements will be collected. Groundwater samples will then be collected from 12 micro-wells including MW-1, MW-2I, MW-3, MW-4, MW-5, MW-6, MW-8, MW- 10I, MW-14R, MW-16, MW-18I and DW-1 and analyzed for CVOCs using USEPA Method 8260.

Baymeadows Place Cleaners NAM Sampling Proposal May 26, 2021

The groundwater sample collected from source area micro-well MW-1 will be analyzed for total recoverable petroleum hydrocarbons (TRPHs) using the FL-PRO method to monitor for potential groundwater impacts from the EOS-PRO amendment that was placed in the source removal open excavation. Additionally, Wood recommends analyzing the groundwater samples collected from micro-wells MW-1, MW-2I, MW-3, MW-5 and MW-13 for natural attenuation parameters including total iron (USEPA Method 6010), ferrous iron (USEPA Method 6010), sulfate (USEPA Methods 375.2/300.0), sulfide (USEPA Methods 9030/SM4500-S2-), nitrate (USEPA Methods 353.2/300.0), nitrite (USEPA Methods 353.2/300.1), total organic carbon (USEPA Methods 9060/SM5310B). It is also recommended that Microseeps be utilized for these same five wells to be analyzed for the dissolved gases methane, ethene and ethane using USEPA Method RSK175 by Advanced Environmental Laboratories in Tallahassee, Florida.

Samples will be shipped overnight under standard chain of custody protocol.

All purge water will be containerized in a 55-gallon drum. The drum will be staged at a secure location onsite pending the analytical results. If the analytical results indicate that CVOCs are all below the hazardous waste concentrations the drum will need to be transported from the site to an approved State of Florida disposal facility otherwise a change order will be submitted for alternative disposal options. The subcontractor for IDW disposal, Capitol Environmental Services, Inc., was chosen on a lowest cost basis and all bids are still valid.

Upon completion of the annual NAM sampling field activities and receipt of the analytical data, Wood will provide an Annual NAM Report detailing the activities and findings of the sampling event and will include updated tables with analytical data and groundwater elevations and figures showing analytical results, groundwater plumes and groundwater flow at the site, as well as conclusions and recommendations. Field notes and laboratory analytical reports will also be included.

The total estimated cost for the proposed activities is $10,579.07. The sampling event will be conducted once a signed Task Assignment has been received.
