Subject: Task Assignment Negotiation Summary HW688 Task Assignment DC004A Annual Groundwater Sampling Sunbeam Laundry & Dry Cleaners, ERIC_ 4612

This Task Assignment authorizes Wood Environment & Infrastructure Solutions, Inc.

to conduct annual groundwater sampling at the above referenced site. All work will be conducted in accordance with the scope of work and cost proposal submitted on June 9, 2022. Specific subtasks are detailed below:

Subtask 1: Project Management

Project management is calculated at 13% of the direct labor costs.

Subtask 2: Annual Groundwater Sampling

This subtask includes labor, laboratory, subcontractor and equipment costs to perform annual groundwater sampling. personnel will collect groundwater from eight (8) microwells to include MW-1, MW-3, MW-4, MW-5, MW-6, MW-8, DW-2 and DW-3.

Each of the samples will be analyzed for chlorinated volatile organic compounds (CVOCs)) using EPA method 8260. Subcontractor costs included the proper disposal of the purge water that is containerized in a 55-gallon drum. The deliverable for this subtask will include an Annual Groundwater Monitoring Report detailing the activities and findings from the sampling event, including field notes, lab reports, disposal documentation, updated tables and figures to include plume map and groundwater flow direction figure.

Subcontractors for purge water disposal were chosen on a lowest-cost basis.

Documentation of the costs is included in the attached CEF, Level of Effort worksheets, and the contractor’s proposal. The final deliverable due date is November 4, 2022 and the completion date for the task assignment is January 4, 2023. The total cost for Task Assignment DC004A is $5,561.47

RE: Cost Proposal to Conduct Semi and Annual Groundwater Sampling

Sunbeam Laundry & Dry Cleaners 3845 Baymeadows Road Jacksonville, Duval County, Florida

ERIC_4612

Task Assignment No: HW688-DC004A FDEP FAC ID No: 16/9701139

Dear Ms. Shirah, Wood Environment & Infrastructure Solutions, Inc., (Wood) has prepared this cost estimate to continue groundwater monitoring activities of four existing monitoring wells at the former Sunbeam Laundry & Dry Cleaner site in Jacksonville, Florida per the letter dated November 22, 2021 and conversations with the FDEP to include two additional monitoring wells, MW-1 and

DW-2.

All sampling activities conducted during this groundwater sampling event will be in accordance with the Standard Operating Procedures (SOPs) described in the original Site Assessment Workplan dated March 17, 2015. The last sampling event at the site took place on August 17, 2021. Figure 3 shows the locations of the existing micro-wells to be sampled.

The proposed scope of work is detailed below:

Subtask 1 – Project Management This subtask includes all tasks associated with managing and scheduling the field events at the site and submitting the deliverables.

Sub Task 2 - Groundwater Sampling Event

Wood will mobilize a two-person crew to the site to conduct the groundwater sampling activities.

Prior to collecting groundwater samples, all 12 micro-wells will be opened and allowed to equilibrate and then water level measurements will be collected. Groundwater samples will then be collected from four micro-wells including MW-1, MW-6, DW-2 and DW-3 and analyzed for chlorinated volatile organic compounds (CVOCs) using USEPA Method 8260 by Advanced Environmental Laboratories in Tallahassee, Florida. Samples will be shipped overnight under standard chain of custody protocol.

All purge water will be containerized in a 55-gallon drum. The drum will be staged at a secure location onsite pending the analytical results. If the analytical results indicate that CVOCs are all below the hazardous waste concentrations, the drum will need to be transported from the site to an approved State of Florida disposal facility.

As a deliverable for this event, Wood will prepare a Groundwater Monitoring Report detailing the activities and findings from the annual sampling event including updated tables and figures.

The figures will include the current contaminant plume based on the most recent analytical data, as well as a figure depicting the groundwater flow at the site.

The total estimated cost for the proposed activities is $5,561.47. The sampling event will be conducted within 30 days of receiving a signed Task Assignment.

Invoice & Deliverable Schedule

Subtask Deliverable Deliverable Due Date Invoice Amount

2 Groundwater Sampling Report 11/4/2022 $5,055.50 + $505.97

*Assuming FDEP tasks project by June 20, 2022 **A detailed Project Schedule is attached.

Based on the results of the sampling events, Wood will make recommendations for future work that may be necessary at the site. Please let me know if there are any questions concerning this proposal.

Sincerely, Wood Environment & Infrastructure Solutions, Inc.
