TO: HW560 Contract File FROM: Walsta Jean-Baptiste, Contract Manager SUBJECT: Imperial Cleaners; Interim Source Removal Implementation; DC037E DATE: February 23, 2021
This Task Assignment authorizes Tetra Tech to implement the approved Interim Source Removal Plan for soil vapor extraction system installation in accordance with the Tetra Tech scope of work, cost estimate form, bid documentation, and schedule received on February 22, 2021.
Subtask 1: Project Management Project management costs are calculated at 12.5 % of the direct labor cost.
Subtask 2: SVE system construction/installation and startup This subtask includes costs for system installation and startup. Based on a review of available FDEP treatment systems, Tetra Tech determined that it was necessary to purchase a system. The equipment procurement handling fee for the remedial system purchase was calculated correctly at $2,500. The subcontractor handling fee for the installation contractor was calculated correctly at 10%. Tetra Tech received three responsive bids for installation but only two responsive bids for equipment purchase and selected the lowest responsive bidder for each.
The cost estimate was corrected to remove redundant forklift and carbon costs. Additionally, ESD Waste2Water costs for equipment were moved to the equipment procurement section of the cost estimate form so that the handling fee would be calculated correctly.
Subtask 3: Remedial Action Startup Report This subtask includes costs for startup reporting.
Subtask 4: System Monitoring – First Month This subtask includes costs for the first month of O&M related to startup.
The utility cost for the first month was removed from this Task Assignment and will be included in a subsequent O&M TA.
The final cost associated with this Task Assignment is $113,703.21 with 228 hours.

INTERIM SOURCE REMOVAL IMPLEMENTATION
FORMER IMPERIAL CLEANERS
10055 NORTH DALE MABRY HIGHWAY
TAMPA, FL 33618
ERIC_4789
The SVE system will include four (4) vertical vapor extraction wells (VEWs) (VEW-1 through VEW-4), utilizing existing MAGS locations. The VEWs will be operated continuously, or as needed, in order to remove residual PCE mass under the building. The extraction well design allows for overlapping of the ROIs, as indicated on Drawing IMP-02. Based on similar SVE designs and performance, an ROI of up to 20 feet can be anticipated.
Tetra Tech will provide construction supervision and management for the installation of the SVE system.
Construction and installation work will be performed by subcontractors. All electrical work, plumbing work, underground utility construction, and excavation will be performed in accordance with Chapters 489 and 553, F.A.C. in addition to local city and county codes. Detailed construction drawings and notes are provided on Drawings IMP-01 through IMP-06. Installation is generally comprised of two parts:
1. Site Preparation, which will include but not be limited to the following:
• Notification of property owner(s) of the estimated construction commencement date;
• Performing necessary utility searches to finalize field location of equipment, piping, and tie-downs;
• Posting of required signs, barricades, and permits; and
2. Construction Sequence, which will generally be as follows:
• Delivery of materials and construction equipment;
• System and well installation coordination;
• Installation and hook-up of electrical supply;
• Installation of extraction wells and transfer piping;
• Installation of surface seal; and
• Delivery and installation of the SVE treatment system (including electrical hook-up)
The SVE treatment system is designed to include a network of four (4) 2-inch vertical VEWs. For the VEWs, existing MAGS (MAGS-01, -02, 04R and 05R) locations that were left in place and capped will be converted to VEWs constructed of 2-inch diameter schedule 40 PVC casing with 3 feet of 0.020 high capacity machine slotted PVC screen from 1 to 4 feet bls.
The 2-inch diameter schedule 80 PVC individual vapor transfer lines from the wells will be field routed to the SVE treatment system skid. The transfer lines from the wells located inside of the building will run towards an existing interior wall and then along the east and north exterior walls, culminating at the alleyway adjacent to the building. From the back or east wall of the building, the transfer lines will penetrate the wall and be run outside to the SVE treatment skid on the north side of the building in the back alley. The transfer lines from VEW-3 and VEW-4, located outside of the building, will be run along back alley wall just outside the north side of the building directly to the equipment skid. Each VEW will be plumbed to the equipment skid individually. The proposed piping layout and equipment skid location are presented on Drawing IMP-03 of the ISR Work Plan.
Tetra Tech will provide construction supervision and management for the installation of the SVE treatment system. Construction and installation work will be performed by Tetra Tech’s subcontractor(s). Subcontractor will follow health and safety procedures and requirements per OSHA specified in Title 29, CFR and warning signs will be posted in accordance with Chapter 62-730, F.A.C.
Following the remedial construction activities, Tetra Tech will make a final inspection of the site and subcontractor’s activities. Inspection and certification by appropriate city officials may be performed, as required by local codes. Upon the project reaching a satisfactory and successful completion, SVE startup activities will be performed
After electrical service is provided, Tetra Tech will perform the initial startup and verify system performance as described below. A startup test will be performed following initial startup of the system in order to confirm design criteria and to establish system operating parameters. The startup sequence will ensure the proper functioning of the SVE treatment system.
In general, the initial startup sequence for the SVE treatment system is as follows:
• Collect depth to groundwater measurements across the site;
• Verify that all sampling valves are in the closed position;
• Open all other valves (e.g. recirculation line, extraction line, and air supply line) to the fully open position;
• Open the dilution air (i.e. bleed) valve to the fully open position;
• Start the blower motor;
• Start closing down the air bleed valve, adjust recirculation valve such that the exerted vacuum is within acceptable ranges;
• Adjust the SVE well network valves at the manifold for individual VEWs to achieve the desired flow and vacuum;
• Monitor the system and make necessary adjustments to achieve steady-state operations;
• Take baseline PID readings and bag samples of the influent, effluent and individual VEWs at startup.
Following the initial startup, testing will be performed to quantify the approximate ROI and to establish optimal system operation. From this limited testing, the operating scheme and set points for the SVE treatment system will be established.
The schedule for completion of this project is attached.

IMPERIAL CLEANERS (FORMER)
COST PROPOSAL - REMEDIAL ACTION CONSTRUCTION
HCR understands the anticipated schedule for system installation is April 2021. However, one issue that may delay the proposed construction schedule is related to permitting with the Tampa and electrical permitting. Prior to initiation of work, HCR has assumed that
Tetra Tech will make all necessary notifications to the site representatives.
Scope of Services – Based on the Village Laundry Bid Sheet
Item - Permits HCR will be responsible for all required licenses, permits, and fees required to complete the scope of work. HCR will work to obtain the required construction and building permits as required by the local agencies. Copies of required permits (as needed) will be presented to Tetra Tech prior to the commencement of any work. Permitting may require
PE signed and sealed construction drawings to be provided by Tetra Tech.
Item – Mob/Demob Includes the mobilization and demobilization for the required equipment, vehicles, and
Maintenance of Traffic (MOT, as needed). In addition, item includes costs for the completion of a Pre-Construction meeting (one day) as well. Tetra Tech will be responsible for the preparation a Site-Specific Health and Safety Plan (HASP).
HCR will establish the work zone with barrels and cones to prevent unauthorized personnel from entering the work zone. HCR will verify underground utility markings prior to initiating work activities. HCR will work diligently around any conflicting subsurface utilities to prevent accidental damage. Note, HCR shall establish the presence of underground utilities in the area of work and monitor all intrusive activities during the performance of this contract to prevent damage to underground utilities.
GPR utility clearance activities will be completed prior to the Pre-Construction meeting.
Based on visits completed to the site, the area appears to have several unmarked underground utilities (gas, electric, water, sewer, etc).
Additionally, if the amount of utilities results in an unsafe trenching activity, a change order for the use of a vacuum truck to work around sensitive areas may be required.
Item - Vertical 2-inch Vapor Extraction Wells (4' deep) (already in-place, capped just below grade, need to be converted with appropriate above-grade piping and connectors, see Drawings IMP-03, IMP-05, & IMP-06) (2)exterior Vapor Extraction wells will require modification reinstallation.
Includes labor and materials to make the direct connections to the four existing buried vapor extraction wells (VEW-1, VEW-2, VEW-3, VEW-4).
**If any of the wells are not suitable for use, the HCR project manager will request a change order to reinstall the wells. The wells will be set to a terminal depth of 4-feet below land surface (ft bls) and constructed with 3-feet of 2-inch diameter 0.010-inch slotted SCH 40 PVC well screen threaded to 1-foot of 2-inch diameter riser.
Pea gravel (1/4-inch) will be poured into the boring annulus to 1-foot bls and a six-inch layer of fine sand will be placed above the gravel pack to create a barrier prior to installation of the grout seal which will be advanced within about six inches of land surface.
Item - Vapor Transfer (Extraction) trenching for VEW-1 AND VEW-2 As described on the Bid Sheet, the exact route for VEW-1 and VEW-2 will be determined in the field prior to the construction activities. Includes labor and equipment (concrete saw cutter, compactor, etc) to complete approximately 25 feet of trenching to reach the other side of the interior wall of the High Security Merchandise Area prior to plumbing from the interior of the T-Mobile store to the exterior of the building.
Item - Vapor Transfer (Extraction) Piping (2-inch Schedule 80 PVC) below grade Includes appropriate Schedule 80 PVC piping and associated connectors from the VEW-
1 and VEW-2 wellheads to underneath the interior wall of the High Security Merchandise
Area.
Item - Vapor Transfer (Extraction) Piping (2-inch Schedule 80 PVC) above grade Includes appropriate Schedule 80 PVC piping and associated connector and ball vales.
Includes an extra 20 ft for VEW-1 and VEW-2, for going up and down the walls as well.
Item - Piezometer Installation 2" dia. (10' total depth w/ 5-ft screen) for High Groundwater Cutoff Switch Includes the labor and materials to install a 2-inch Schedule 40 PVC piezometer well with
5 feet of screen to a maximum of 10 ft bls. For costs savings, HCR will install the non-permitted well via hand auger to at least two feet into the water table. As was discussed, only two feet into the water table are required for the water level to be placed in the well.
Item - High Water Level Sensor Conduit (3/4-inch Schedule 80 PVC) It is assumed Tetra Tech will provide the required water level senor. HCR’s subcontracted electrician will complete the tie of the level switch to the skid system.
Item - Vacuum Rated Flex Hose - 2" with cam lock fittings to connect VEW line to trailer Includes estimated vacuum hosing and camlock connectors.
Item - Electrical Work (3-Phase Power Drop Weather Head, Riser, Meter Can, Misc.
Wiring, Trenching, etc.)
The electrical work and required permitting will be completed by HCR’s subcontracted electrician (ABP Electric, Inc.). Based on site visits and discussions, the electrician and
HCR have determined the best course of action to provide 3-phase power to the system will be to trench approximately 80-feet from the planned SVE skid location to the power transformer. HCR will complete the trenching activity (saw cut a 80-foot long trench approximately 1-foot wide [asphalt overlain concrete]), excavating the trench to approximately 2.5 ft bls, setting the conveyance electrical conduit approximately two feet below grade, backfilling/compacting the trench, resurface and finish with 3000 psi concrete with fiber, and disposing of all concrete/asphalt. The selected electrician will coordinate the utilities connections. It assumed the existing transformer in the alley way has sufficient capacity for the operation of the system. Should the existing transformer’s capacity not be sufficient, a change order may be required to run power to the system.
Prior to initiating the work on site, during the pre-construction meeting, HCR and the electrician will verify the capacity of the transformer with the power company.
Item - SVE Treatment Skid Installation (system including hurricane anchors and four 6" concrete filled steel bollards) Includes labor and equipment (concrete corers, hammer drill, generators, etc) to complete the installation of the internal PVC piping from VEW-1 and VEW-2 to the system skid
(through the buildings wall) located outside, installation of the four concrete filled bollards, construction of the SVE manifold, and installation of two hurricane tie-downs (based on size of skid).
Item – Site Restoration The disturbed areas will be restored with concrete, concrete patches for the vapor wells located inside the building and concrete for the proposed trenching. The vinyl floor will be completed with like-for-like vinyl flooring. Outside trenching in paved areas with be restored with approximate 6 inches of 3,000 pounds per square inch (psi) concrete to existing grade. All concrete debris will be transported and disposed offsite as general construction debris. Should the concrete thickness be greater than 6 inches, additional costs may be applied under a change order for added cutting, disposal, and restoration.
*During the pre-construction meeting, HCR will discuss if it’s feasible to store a 20-yard roll-off on the T-Mobile Property during the construction activities. If not, HCR will utilize a pull behind trailer to dispose of the asphalt/concrete and other construction debris generated.
Item - Start-Up A separate day of start-up assistance has been included to conduct remedial system start-up and troubleshooting with Tetra Tech. A representative from HCR and the electrician will be in attendance.
Item – Per Diem, Per Man Per Diem included for a three-man crew for the ten days to complete the system installation. Per Diem is included for the following work: two days to complete the trenching in the interior of the T-Mobile building, five days to complete the piping from the wellheads to the remediation equipment area, constructing the manifold, securing the
SVE skid, installation of the traffic bollards, and site cleanup, followed by three days to complete the trenching for the electrical service and to oversee the installation of the electric service.
The scope of work will be completed in a total of 14 working days comprised of one day for the Ground Penetrating Radar event, one day for the pre-construction meeting, 10 days for the construction activities outlined above, one day of startup assistance, and one day to oversee the site restoration activity (if unable to be completed in conjunction with other work [vinyl flooring]).
HCR’s quote is based on the information provide by Tetra Tech. Upon the issuance of a
Letter of Authorization, HCR will begin to coordinate the installation of the SVE remedial system as described.
Terms and Conditions 1.) Our price assumes that HCR will have unrestricted access to the work area during normal business hours. If needed, HCR can adjust the pricing for weekend hours to minimize delays to daily site operations.
2.) Unforeseen delays not caused by HCR which require excess stand by time or excess time on site will require additional equipment and labor charges. Any unforeseen conditions necessitating a change in scope shall be presented to the
Tetra Tech in writing using a “Construction Change Order” form.
3.) Our price is contingent on mutually agreeable terms and conditions with payment terms of Net 30.
