About this file
This is a Task Assignment Notification Change Order Form (Change Order #1) for DEP Contract No. HW560 between the Florida Department of Environmental Protection (FDEP) and Tetra Tech, Inc. The contract pertains to supplemental site assessment activities at the Port of Fort Pierce, also known as the Former King Maritime Group, located in Fort Pierce, Florida (Site ID ERIC_15269). The change order was processed by DEP Contract Manager Kelly Crain and DEP Task Manager Megan R. Johnson, with contractor representative Gerry Walker, P.G., and was executed in January 2021.
The change order modifies Task Assignment BF602B for supplemental site assessment activities funded through the State & Tribal Response Program (SRP18 Grant), a federal cooperative agreement administered by FDEP on behalf of the U.S. Environmental Protection Agency. The modification reflects actual field conditions encountered during Phase II assessment work and includes the following adjustments: an increase of 8 hours for a Scientist IV to coordinate onsite sampling activities (Subtask 3), resulting in a net cost increase of $342.24; a decrease in subcontracted drilling costs of $550.00 due to incorporation of two pre-existing monitoring wells, reducing new well installations from four to two; and a decrease in laboratory costs of $1,084.00 due to fewer samples collected than originally proposed. The overall result is a cost decrease of $491.54, reducing the total task amount from $36,372.08 to $35,880.54. The final deliverable due date for Subtask 4 (Supplemental Site Assessment Report) was extended from January 15, 2021 to February 5, 2021, with the overall period of performance extended by 18 days to March 5, 2021.

SUBJECT: Negotiation Summary – SRP18 Grant
Task Assignment (TA) Change Order #1 – TA BF602BC1 Port of Fort Pierce aka: Former King Maritime Group Site ID # ERIC_15269 Supplemental Site Assessment Activities
This Task Assignment Change Order (TACO) is for an increase of 8 hours for a Scientist IV, a decrease in subcontracted drilling cost, and a decrease in laboratory cost, resulting in an overall decrease of costs for Subtask 3 – Field Activities; this led to a slight increase of 0.7 hours to Subtask 1 – Project Management; and a time extension to the deliverable due date for Subtask 4
– Supplemental Site Assessment Report. A time extension to the overall Period of Performance was given, as well. This work was conducted at the Port of Fort Pierce aka: Former King Maritime Group site, located at 101 Port Avenue A, Fort Pierce, Florida 34950. The services under this task are being conducted using federal funds from the State & Tribal Response Program (SRP) cooperative agreement that is administered by the Department on behalf of the U.S.
Environmental Protection Agency (EPA).
Subtask 1: Project Management An increase in labor expended increased the number of project management hours by 0.7 hours. This resulted in an increase of $131.94 for this Subtask.
Subtask 2: QAPP/Work Plan, and HASP This TACO does not include any costs changes associated with this Subtask.
Subtask 3: Supplemental Site Assessment Activities On November 30, 2020, Tetra Tech sought Department approval to include eight (8) hours of time for a Scientist IV to assist in the coordination of onsite sampling activities due to this individual’s prior knowledge of the site. The Department approved this request on December 2, 2020, and that approval email is attached.
Two pre-existing monitoring wells were incorporated into the groundwater monitoring network, which reduced the number of new wells from four (4) to two (2), reducing the subcontracted drilling cost. In addition, fewer samples were collected than those included in the original cost proposal due to an overestimation of number of TRPH samples needed prior to workplan development.

This resulted in a decrease in laboratory costs These changes resulted in an overall decrease of ($623.48) for this Subtask.
Subtask 4: Supplemental Site Assessment Report Included in this TACO is a deliverable due date time extension for this Subtask. This TACO does not include any costs changes associated with this Subtask.
Summary The Department’s SRP Contract Manager and Tetra Tech’s Contract Manager have both agreed to the above changes. Due to a rounding error in Excel, the Cost Actual LOE spreadsheet states an overall project cost of $35,880.56, a decrease of ($491.52), when it is actually $35,880.54, a decrease of ($491.54), as shown in the Delta LOE spreadsheet. Therefore, all changes noted above represent a total overall cost decrease of ($491.54). The changes were noted in the TACO Request received from Tetra Tech, Inc. on January 12, 2021. The final cost associated with this TA decreased from $36,372.08 to $35,880.54. Documentation of the changes is included in the attached TACO Request, the Delta LOE spreadsheet, and the Cost Actual LOE spreadsheet, the TACO Form for BF602BC1 prepared by the Department, and the December 2, 2020 Department approval of additional field hours email. The final deliverable for Subtask 4 – Supplemental Site Assessment Report, must be received by the Department no later than February 5, 2021.

Cleanup Programs Consolidated Contract HW560
Subject: Change Order Request – Phase 2, Targeted Brownfield Assessment at Former King Maritime Group (AKA: Port of Fort Pierce), Fort Pierce, St. Lucie County, Florida
Dear Ms. Johnson:
Tetra Tech is providing this change order request to the Florida Department of Environmental Protection (FDEP) to document changes in cost associated with scope changes that occurred during the field work (Subtask 3) for the second phase of Targeted Brownfield Assessment activities at the Former King Maritime Group Site at 101 Port Avenue A, Fort Pierce, Florida. We are also requesting a time extension for completion of the final report (Subtask 4) in this change order.
The scope changes necessitating the change order are described below and in the attached Change Order and Change Order Delta workbooks.
Subtask 1 - Project Management: Increase (0.7 hour) due to Subtask 3 labor increase.
Subtask 2 - Work Plan & Health and Safety Plan (HASP): No changes.
Subtask 3 – Soil Sampling, Well Installation, and Groundwater Sampling:
A. Increase in LOE cost. FDEP approved the inclusion of LOE for a Scientist IV (SO) to assist in the coordination of onsite sampling activities with the onsite representative from Derector and the Tetra Tech Field Operations Lead. Mr. Sean Ouellette led the previous sampling effort and his coordination greatly facilitated access to the site and locating the sampling stations. This request includes eight (8) hours of his time that were not included in the original cost proposal.
B. Decrease in subcontracted drilling cost. Two pre-existing monitoring wells were incorporated into the groundwater monitoring network, which reduced the number of new wells from four (4) to two (2). This reduced the drilling cost by $550.00. The invoice from the drilling subcontractor is attached.
C. Decrease in laboratory cost. Fewer samples were collected than those included in the original cost proposal. This was due to an overestimate of number of TRPH samples needed in the original scoping prior to workplan development. Fewer surface soil samples were also collected from the 0 to 6-inch below land surface soil interval at some locations due to concrete pavement and pavement base material that occurred in Site Areas B and C. The changes in numbers of samples and resulting cost difference are shown in the Change Order Delta LOE 3 Worksheet. Actual samples collected are documented in the attached copies of the sample “Chains of Custody and sample receipts.
Subtask 4 - Report: Due to several factors, the field work was conducted approximately three weeks later than anticipated in the original schedule for the task assignment. The original schedule did not account for time needed to obtain a right of way permit from the City of Fort Pierce for background sampling or scheduling weekend work with the driller to work around ongoing construction work on the former port site. Tetra Tech requests a change in the final deliverable due date from January 15, 2021 to February 5, 2021.
Change Order Amount These changes result in a $491.54 reduction in cost for this task assignment. The requested new task assignment amount is $35,880.56.
If you have any questions, please call myself or Richard Hicks (850) 385-9899.
Sincerely yours, Gerald Walker, P.G.

Good morning Rick, Thanks so much for the updates and for checking on adding some hours for Shawn. I spoke with Kelly and that is no problem, we appreciate Shawn’s knowledge of the site and relationship with the site manager. Looking forward to seeing the ROW permit package and hearing any feedback that comes from the Friday site visit. Hope you all had a great Thanksgiving!
Thanks, Megan

Hi Megan. I just wanted to give you a brief update on what’s going on.
-We have a schedule for doing the drilling and sampling work. Drilling and soil sampling will occur on 12/18-19 and we will do the groundwater sampling and surveying early in the following week, week of 12/21.
-Our folks will be onsite this Friday to meet with the site rep at Derektor and do the utility locate work. Chris Adkison of our Cocoa office is going to oversee/conduct the drilling/sampling but I asked Shawn Ouellette from Stuart who did the last phase of sampling to go with Chris for the site visit, to help locate the onsite soil borings and coordinate with the site rep at Derektor. Shawn has a good relationship with the site manager and I didn’t want any hitches to occur in communication when we go into the site to do our work in the construction area. With your permission, I may request adding a few hours for Shawn.
-We will submit our ROW permit package to the City Engineer for locations on City ROW after the utility locate and be ready to go with work on ROWs by 12/18. I will send you a copy of the permit package.
-Also, there are two existing wells at the site that we discovered in a visit a couple of weeks ago that may serve our purposes and allow us to eliminate one of the wells we were planning to install, which would reduce some cost.
Let me know if you have any questions or need other information. Thanks. Have a great week.
