Tetra Tech, Inc.

1558 Village Square Blvd., Ste. 2, Tallahassee, FL 32309-2478

Tel 850.385.9899 Fax 850.385.9860 www.tetratech.com source area, and one located south of the building to serve as a background location.

The background soil sample is away from the potential source area where soil may have been impacted. At the three proposed locations, two soil samples (surface [0 - 2 foot depth and subsurface [2 o 4 foot depth, or 1 foot above water table]) will be collected for laboratory analysis of the 22 Target Analyte List of metals per Method 6010B plus Mercury (Hg) per Method 7470; Target Compound List Volatile Organic Compounds (VOCs, per Method 8260B); Cyanide (Method 9012B), and % moisture. In addition, Quality Control (QC) samples are included in the cost proposal for your consideration.

These include one field duplicate and one equipment rinsate blank (for all analytical methods identified above). FDEP guidance (FC1000 in https://floridadep.gov/dear/quality-assurance/content/dep-sops) defers to programs on selection and number of field duplicate samples needed for a given project, although equipment and trip blanks are required. Temperature blanks will be carried in all coolers.

A CLP-Like Level IV analytical data package will be provided by the subcontracted analytical services laboratory (TestAmerica) for their deliverable. There is no additional cost to FDEP for the Level IV deliverable. Soil sample aliquots for all analytical categories except volatile organics will be collected using pre-cleaned stainless-steel hand augers and homogenized in Pyrex bowls (or approved alternative) prior to transfer to sample containers. Each aliquot for volatile organic compound analysis will be collected directly from the hand auger bucket using a syringe-type core sampler and transferred to sample vials according to U.S. Environmental Protection Agency (USEPA) 5035 methodology. Excess soil removed during soil sampling will be returned to the boreholes after sampling.

Monitoring well installation and abandonment. Based on the available data the site-specific direction of groundwater flow is inconclusive. Data from the Preliminary Assessment (PA), indicates a local groundwater flow to the south, other sources indicate a westerly flow direction. Therefore, the monitoring well placement attempts to account for this with the installation of four temporary monitoring wells to evaluate upgradient and downgradient groundwater quality at the potential contaminant source area on the north side of the facility.

Prior to the monitoring well intrusive activities, Tetra Tech will have the monitoring well boring locations surveyed for buried utilities by a utility locator service (GeoTek, see attached quote). The monitoring wells will be installed as small diameter (micro) PVC wells installed by a direct-push drilling contractor (selected contractor is Cascade; see attached quotes). Each well will be completed to an approximate depth of 12 feet below land surface (bls). Actual well depths and screen placement will be determined based on lithology and depth to groundwater encountered in a continuous core hole advanced at each well location. Screens will be placed so that the upper 1/3 of the screen is above the water table, if allowable by the water table. The estimated depth to groundwater reported in the PA was approximately 2 feet bls. Each well will be constructed of 1-inch-diameter PVC casing with a 10-foot-long pre-packed well screen, with a fine sand seal above the well screen. Clean bagged sand will be used to fill the annular space above the seal. As soon as the wells are installed, they will be developed by pumping and then purged for sampling.

After Tetra Tech completes sampling and a top-of-casing/water level survey, the wells will be abandoned in place by grouting using the tremie method, with the well casings pulled or cut off bls, and the surface restored to the pre-existing condition. This method for well installation generates no drill cuttings to collect as investigation-derived waste (IDW).

Development and purge water will be allowed to infiltrate into the soil at the well sites.

Based on time estimates from the drilling contractors, it is estimated that the soil boring, Tel 850.385.9899 Fax 850.385.9860 www.tetratech.com monitoring well installation, and well development work will be completed in one day.

Abandonment of the wells will occur after the groundwater samples are collected, the wells are surveyed and the depths to groundwater are measured. Tetra Tech will request a change order to reduce drilling cost if actual invoiced amount is different than the amount included in this cost proposal.

Groundwater sampling. The four temporary wells will be sampled during the site inspection shortly after they have been developed, with no specific waiting period between development and sampling. Each well will be purged with a peristaltic pump and Teflon tubing using low-flow sampling procedures and samples will be collected for laboratory analysis of 22 Target Analyte List of metals per Method 6010B plus Mercury (Hg) per Method 7470; Target Compound List Volatile Organic Compounds (VOCs, per Method 8260B); and Cyanide (Method 9012B). In addition, Quality Control (QC) samples are included in the cost proposal for your consideration. These include one field duplicate and one equipment rinsate blank (for all analytical methods identified above), and one trip blank (for VOCs only).

Other field activities. A hand-held global positioning system (GPS) will be used by Tetra Tech staff to survey latitude and longitude coordinates of the sampling stations. In addition, all monitoring well tops of casings will be surveyed by Tetra Tech staff to a common onsite datum and depth-to-groundwater measurements will be taken during the study to document groundwater flow direction.

The field personnel will mobilize from Tetra Techpg Tampa, Florida office, arriving on site to conduct a reconnaissance and meet with the utility contractor to locate buried utilities in advance of the sampling work and returning to the site to meet the driller and conduct the monitoring well installation and sampling activities. The proposed hours for this subtask include 2 hours for a Scientist V (RH, for coordination and deliverable review), 32 hours for the Scientist IV (GB) based in Tampa (for mobilization/demobilization and field work) and 2 hours for the Engineer II (MQ, for the field task deliverable preparation). The staff member ]b LYhfU LYW\pg Tampa office has past experience in working cb ><=Hpg Consolidated Contract and has extensive experience in CERCLA and Brownfields assessment work. The Tampa office is also located within a 1-hour drive of the site.

The field work is anticipated to be completed in a two-day period, which includes mobilization and a site walkover to establish the sampling stations and locate utilities on one day and monitoring well installation, sampling and demobilization on the second day.

Subtask 4 - Report:

The level of effort assumes 2 hours for a Scientist V (GW), 4 hours for a Scientist V (RH), 36 hours for an Engineer II (MQ), and 5 hours for figure preparation (JM). Report tasks include preparation of draft and final reports documenting the field activities, site conditions, and results

Tel 850.385.9899 Fax 850.385.9860 www.tetratech.com of the sampling effort. The report will adhere to the recently developed template for streamlined CERCLA Site Inspection Reports and will include information on site background and observations and findings of the SI. RE: Utility Locating Services – Bore Locations @ 4407 Buena Vista Ln, Holiday FL

Mr. Hicks, GeoTek Services is pleased to respond to your request for Utility Locating services in Holiday, Florida. We will scan a 10’ x 10’ area around the 4 proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as vehicles, fencing, or buildings. We will scan as close as reasonably possible to fixed objects such as obstructions, landscaping, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Best Regards, Martin (Bud) Connor GeoTek Services, LLC
