About this file

This document is a cost proposal from Tetra Tech, Inc. submitted to the Florida Department of Environmental Protection (FDEP) Waste Cleanup Programs under the Consolidated Contract HW560. The proposal, dated October 27, 2017, addresses environmental remediation services for the Avalon Inn Property located at 443 4th Avenue North in St. Petersburg, Pinellas County, Florida. The engagement involves three primary tasks: project management, monitoring well installation with groundwater sampling and analysis, and preparation of a groundwater assessment report.

The scope of services includes installation of three monitoring wells (two shallow wells at 17 feet below land surface and one deep well at 35 feet below land surface) using Direct Push Technology, with wells completed as flush-mount installations with 8-inch diameter bolt-down well vaults. Groundwater sampling will be collected from six existing wells and three newly installed wells, with nine samples analyzed for petroleum-related constituents via expedited 3-day laboratory turnaround using USEPA methods for volatile organics, polynuclear aromatic hydrocarbons, lead, and total petroleum hydrocarbons. Additional baseline samples from two existing wells will be analyzed for dissolved iron, manganese, chemical oxygen demand, biological oxygen demand, and methane. The project will be completed on a fixed-price basis with invoicing at the completion of Task 2 and Task 3. The estimated schedule anticipates project completion within ten calendar weeks from task assignment, with work commencing upon receipt of the signed task order. Detailed cost estimates and subcontractor bids, including a winning bid from DrillPro LLC for monitoring well installation, are provided in the attached spreadsheets.

Subject: Revised Fourth Cost Proposal – Monitoring Well Installation and

Groundwater Sampling at the Avalon Inn Property located at 443 4th Avenue North St Petersburg, Pinellas County, Florida

Dear Ms. Kruchell:

Tetra Tech is pleased to submit this revised cost proposal including the attached Contract HW560 Cost Estimate Spreadsheet for: monitoring well installation, groundwater sampling, and reporting for the Avalon Inn Property located at 443 4th

Avenue North St Petersburg, Pinellas County, Florida. This letter and spread sheet has been revised for expedited laboratory analysis. The field services, which include associated travel and equipment, will be provided by a 1-person crew. Services provided in this proposal will be completed as three separate tasks and include the following activities:

Task 1 - Project Management: Project Management includes managing the project scope, schedule, and budget; and procurement of subcontractors. The level of effort is based on the HW560 Contract requirements and assumes 9 hours for Task 1.

Task 2 – Monitoring Well Installation; and Groundwater Sampling and Analysis:

Monitoring Well Installation o One experienced Tetra Tech Geologist will observe and document installation of monitoring wells that will be installed using Direct Push Technology (DPT). Two shallow monitoring wells will be installed to 17 feet beneath the land surface (bls) (note; this is the depth of existing shallow monitoring wells) and a deep monitoring well will be installed to 35 feet bls. The shallow wells will be constructed of nominal 1.5-inch diameter, pre-packed schedule-40 polyvinyl chloride (PVC) with 10 feet of

0.10-inch pre-packed slotted screen. The deep well will be constructed of nominal 1.5-inch diameter, pre-packed schedule-40 PVC with 5 feet of 0.10-inch pre-packed slotted screen. A PVC end-cap or well point located at the bottom of each well. Each monitoring well will be completed as a flush mount well with and 8-inch diameter bolt down well vault. Upon completion, the monitoring well will be developed by over pumping with a peristaltic pump until it produces relatively sediment free non-turbid water.

The top of casing elevations of the new monitoring wells will be surveyed relative to the existing monitoring wells by the Tetra Tech Geologist. The level of effort (20 hours) assumes: the Geologist will have two hours of fieldwork preparation time prior to the field work; will mobilize-demobilize from the Tallahassee office (eight hours); and will have 1-1/2 days onsite (one 10-hour day and one 5-hour day onsite). The additional ½ day on site is required to allow the required 24 hours between the well installation and development. The scope of work and quote for monitoring well installation is provided in Attachment A.

o The two hours of fieldwork preparation time by the Geologist is to have underground utility locations marked, obtain field equipment, and decontaminate field equipment.

Groundwater Sampling and Analysis o One experienced Tetra Tech Sampler will collect groundwater samples in accordance with FDEP standard operating procedures from the six existing wells and the three newly installed wells. The groundwater sampling event will occur approximately one week after well installation to allow for stabilization of the aquifer conditions. The level of effort (30 hours) assumes: the Sampler will have two hours of fieldwork preparation time prior to sampling the monitoring wells (order sampling equipment and laboratory analysis, and check the sample bottles); will mobilize-demobilize from the Tallahassee office (eight hours); and will require two 10-hour days onsite to sample the monitoring wells.

o Nine groundwater samples from existing and newly installed monitoring wells plus QA/QC samples will be analyzed by an offsite laboratory for petroleum related constituents including: volatile organics (USEPA Method 8260), polynuclear aromatic hydrocarbons (USEPA Method 8270/8310), lead (USEPA Method 6010), and total petroleum hydrocarbons by Florida-Petroleum Residual Organics (FL-PRO). As per FDEP’s request, the sample analysis will be expedited to a 3-day turnaround.

o Two groundwater samples, collected from existing monitoring wells MW-2 and MW-4, will be analyzed for total dissolved iron and manganese (USEPA Method 6010), chemical oxygen demand (SM522), biological oxygen demand (5 day) (SM5210), and methane (USEPA Method 3810) to baseline these parameters for possible in-situ remedial alternatives for groundwater. As per FDEP’s request, the sample analysis will be expedited to a 3-day turnaround for all analysis except biological oxygen demand which requires 5-day for completion.

o The cost for laboratory analysis is provided in HW560 Cost Estimate Spreadsheet in Attachment B.

o Field forms and a field log book will be used to document the in-situ tank closure activity, monitoring well installation, and groundwater sampling activities completed by the Tetra Tech Sampler.

Task 3 – Groundwater Assessment Report:

o Draft and final reports will be prepared that describe the in-situ tank closure activity, and monitoring well installation and groundwater sampling results. The report will evaluate current site groundwater conditions and potential in-situ remedial alternatives that could be implemented for the petroleum-related constituent plume. The report will include tables and figures that provide a comparison of the soil sample analytical results to Groundwater Cleanup Target Levels per Chapters 62- 780, and 62-777, Florida Administrative Code.

o The report will be prepared at the Tetra Tech Tallahassee office (Review:

Scientist V, 4 hours; Document preparation: Engineer V, 8 hours, Scientist IV, 4 hours, and Engineer I, 58 hours) with support from the Jacksonville office (GIS figure preparation, 4 hours and Project Assistant editing & publication, 4 hours).

o An electronic copy of the draft Completion Report will be sent to FDEP by email or placed on the FDEP FTP site for FDEP review. FDEP comments received on the draft document will be addressed and a final document will be emailed or placed on the FDEP FTP site.

o Following FDEP approval of the electronic version of the final document, Tetra Tech will submit to FDEP two paper copies (one bound and one unbound) and two electronic copies on compact disk (CD) of the Final Report.

1.0 INTRODUCTION

Tetra Tech, Inc. (Tetra Tech) is assisting the Florida Department of Environmental Protection (FDEP) in investigating Avalon Inn Property is located at 443 4th Avenue North in St. Petersburg, Pinellas County, Florida (Figures 1 and 2). This investigation will focus on determining the nature and extent of petroleum-related constituent contamination in groundwater that are associated with past site operations. The work described in this specification will be conducted under the FDEP Contract HW560.

1.1 DEFINITIONS

Tetra Tech Site Representative - The on-site Tetra Tech employee(s) monitoring the implementation of this specification by the Subcontractor.

Subcontractor - Provider of the services defined in this specification.

Project Manager - The Tetra Tech Site Manager responsible for implementing and executing the work by the Subcontractor as described in this specification.

Investigation Derived Waste (IDW) - Waste materials generated by the Subcontractor while working on the

Site. Such materials include, but are not limited to groundwater, soil, decontamination fluids, used personal protective equipment (PPE), and general waste.

1.2 SITE DESCRIPTION

The subject site has an abandoned steel underground storage tank (UST) that is reported to be approximately 500-gallons. The UST is reported to have been used for storing heating oil associated with a former fuel oil furnace which was installed in the basement of the Avalon Inn building. LES reportedly found a permit that indicated a fuel oil furnace being installed on-site during 1948. Based on this information LES assumed that the tank was installed during 1948 and was subsequently in service for approximately 30 years.

1.2.1 Location of Work

The Avalon Inn Property is located at 443 4th Avenue

North in St. Petersburg, Pinellas County, Florida.

1.2.2 Description of Work Area

The Avalon Inn Property site is an out building to an active hotel. The site consists of pavement, except for where the building is located. There is one access point to the site off the unnamed alley on the north side of the property. The site can be accessed from the unnamed alley on the north side of the property via a gate which the property owner stated they would remove for site access.

-3-

Drill and install up to 2 shallow water table zone monitoring wells (MW6 and MW7 to total depths estimated at 17 feet each); and one deep monitoring well (MW5 to total depth estimated to be 35 feet) (Figure 3). The total linear footage is estimated at 69 feet.

Construct flush mount, bolt- down surface well completions for each monitoring well.

Develop each shallow and deep monitoring well.

Containerize and move all IDW and stage 55-gallon DOT approved drums at designated staging area. The subcontractor will be responsible for providing appropriate equipment to manage the

IDW.

Restore site to similar previously existing conditions as appropriate.

Demobilize.

1.3.1 Subsurface Conditions

The lithology encountered by the soil borings typically consisted of: a light grey fine sand to tan fine sand to 5 feet below land surface (bls); a light brown fine sand to dark brown fine sand with increasing silt from

5 to 11 feet bls, and a dark brown silty fine sand or fine sand with silt from 11 feet bls to the explored depth of 20 feet bls. Groundwater was encountered at approximately 8 to 10 feet bls during previous sampling activities.

This description is provided to familiarize the Subcontractor with the general subsurface conditions at the

Site. The description is not comprehensive and may not be complete. Moreover, the subsurface conditions encountered by the Subcontractor may differ from those stated in this section. The Subcontractor, and not

Tetra Tech, is responsible for assessing the subsurface conditions at the Site when scoping, estimating, and implementing the services described in this technical specification.

1.3.2 Potential Hazardous Exposures

In general, the potential hazards are petroleum-related constituent compounds. Historical documents indicate that petroleum-related constituents may be present in soils and groundwater at the site.

Assessment findings indicate that subsurface soil in the vicinity of the 500 gallon UST has been impacted by TRPH at concentrations that exceed Residential and Leachability to Groundwater SCTLs. In addition, a petroleum-related constituent plume exist in the site groundwater (Figure 3). The petroleum-related constituent plume appears to consist of xylenes, 1-methylnaphthalene, 2-methylnaphthalene, naphthalene, and TRPH. Although lead was detected at concentrations exceeding its GCTL, it is believed that this is due to sample turbidity. The constituents in the petroleum-related constituent plume do not exceed their

NADCs. The petroleum-related constituent plume appears to be migrating towards the southwest based on the groundwater flow at the site.

1.3.3 Services Provided by the Engineer

To clarify the Engineers’ obligations during this investigation, the services to be provided by the Engineer are summarized below.

Select the locations and depths of the borings and monitoring wells. Although it is not anticipated, the Engineer reserves the right to reduce or increase the number of boring and monitoring well locations and/or soil samples in the program.

Facilitate the Subcontractor's right-of-access to the sampling locations.

Coordinate location of underground utilities.

The use of mud in drilling operations requires approval by the Tetra Tech Site Representative prior to use.

Mud will be a mixture of bentonite and potable water. No other additives may be used in the mud.

Drilling fluid levels will be maintained at the top of the casing when drilling below the water table. This includes when drilling tools (i.e. drill rods) are being removed from the casing. Drilling fluids will be re-circulated and changed at the discretion of the Tetra Tech Site Representative. Drilling fluids will be containerized as directed by the Tetra Tech Site Representative at the completion of each hole.

The Subcontractor will provide all potable water for drilling and related decontamination activities and will provide sufficient quantities of potable water so as not to impede drilling progress.

2.10 MONITORING WELL INSTALLATION

The Subcontractor will use a DPT 77 or 80 series drill rig or similar type drill rig to install and construct 4 shallow water table monitoring wells to a total depth of approximately 20 feet bls (total linear footage estimated at 80 feet).

The shallow monitoring wells will be constructed of nominal 1.5-inch diameter, schedule-40 PVC with 10 feet of 0.01-inch slotted screen and PVC end-cap or well point at the bottom of each well. The deep monitoring well will be constructed of nominal 1.5-inch diameter, schedule-40 PVC with 5 feet of 0.01-inch slotted screen and PVC end-cap or well point at the bottom of each well.

The outside annular space of each well screen and adjacent formation sands will be filled with 30/45 silica sand to 2 feet above the well screen. The deep monitoring wells will have a bentonite seal, a minimum 1 foot, placed above the sand pack.

Each monitoring well will be properly grouted above the sand pack or bentonite seal using neat Type II

Portland cement. A tremie pipe will be used to install filter packs and grout. The Tetra Tech Site representative will determine well depth and screen setting in the field for each well.

Backfill will be constantly monitored by the Subcontractor and the Tetra Tech Site Representative during the well installation process by means of a weighted steel or plastic tape. The drill casing will be removed from the borehole in increments as backfilling progresses so as not to cause caving of the borehole walls on top of well materials.

Each monitoring well will be completed as a flush mount well with and 8-inch diameter, bolt-down well vault.

The well vaults will be emplaced in 6-inch thick, 2-feet by 2-feet flush mount high strength concrete pad so
