DATE: July 12, 2021

This Task Assignment authorizes Wood Environment & Infrastructure Solutions, Inc. to conduct site assessment activities, as outlined in the May 20, 2021 (Rev. June 23, 2021) Cost Proposal to Conduct Supplemental Site Assessment. Specific subtasks are detailed below.

Subtask 1: Project Management Project management is calculated at 12.5 percent of the direct labor.

Subtask 2: Groundwater Monitoring Wood Environment & Infrastructure Solutions, Inc. will conduct supplemental site assessment activities including annual natural attenuation monitoring (NAM) groundwater sampling and replacement of a damaged monitoring well (MW-14). Wood will conduct groundwater sampling in accordance with FDEP SOPs at 16 micro-wells:

MW-1, MW-2I, MW-3, MW-4, MW-5, MW-6, MW-7, MW-8, MW-10I, MW-11I, MW-12,

MW-13, MW-14, MW-16, MW-17 and DW-1. Samples will be analyzed for chlorinated volatile organic compounds (CVOCs) using USEPA Method 8260 by Advanced Environmental Laboratories in Tallahassee, Florida. IDW disposal is anticipated during this event and has been included under this subtask. The contractor was unable to obtain a third IDW disposal quote due to a lack of response from Perma-Fix Environmental Services, Inc. Additionally, samples collected from micro-wells MW-2I, MW-3, and MW-5 will be analyzed for natural attenuation parameters including total iron, ferrous iron, sulfate, sulfide, nitrate, nitrite, total organic carbon. These three wells will also be analyzed for the presence of methane, ethene and ethane using USEPA Method RSK175. Monitoring well MW-1 will also be analyzed for total recoverable petroleum hydrocarbons (TRPHs) using the FL-PRO method. Upon receipt of the data, the contractor will generate an Annual NAM Report detailing the field activities, procedures, and analytical results.

Wood’s time, labor and subcontractor costs for these activities have been reviewed and are appropriate.

The level-of-effort and final cost of $11,073.78 is reasonable for the scope of services.

Task completion date is December 31, 2021.

RE: Cost Proposal to Conduct Supplemental Site Assessment Baymeadows Place Cleaners 9551 Baymeadows Road Jacksonville, Duval County, Florida FDEP FAC ID No: 16/9502088

ERIC_4558

Task Assignment No: HW559-DC002I Wood Project No: 6090170093

Dear Mr. Meyers, Wood Environment & Infrastructure Solutions, Inc., (Wood) is pleased to provide this cost proposal to conduct supplemental site assessment activities including replacing a damaged monitoring well and pad and to conduct the annual natural attenuation monitoring (NAM) groundwater sampling event at the Baymeadows Place Cleaners site located in Jacksonville, Florida (Figure 1). Wood conducted a complete groundwater sampling event at the site in May 2020 which included collecting groundwater samples from 19 micro-wells. Based on the results of the groundwater sampling activities, Wood prepared an Annual Groundwater Monitoring Report dated July 29, 2020. The May 2020 analytical results obtained from this sampling event indicated that the groundwater plume appears to have shrunk in size for the second sampling event in a row and remains on the Baymeadows Place shopping center property. Based on the reduction of the groundwater plume during the last two sampling events, Wood recommended resampling micro-wells at the site to obtain current groundwater conditions and prepare a Remedial Action Plan to address groundwater contamination. While tetrachloroethene (PCE) concentrations appear to be trending down, daughter products in some wells have increased.

In the May 2020 sampling event, six micro-wells (MW-2I, MW-3, MW-4, MW-5, MW-6 and MW-

7) reported PCE or vinyl chloride at concentrations above their groundwater cleanup target levels (GCTLs) or natural attenuation default concentrations (NADCs). Based on the fluctuating groundwater analytical results at the site, Wood recommends sampling the following 16 micro-wells at the site: MW-1, MW-2I, MW-3, MW-4, MW-5, MW-6, MW-7, MW-8, MW-10I, MW-11I, MW-12, MW-13, MW-14, MW-16, MW-17 and DW-1. These micro-wells need to be sampled again to verify sample results from the last two sampling events to ensure that the groundwater plume is delineated. This cost proposal includes the reinstallation of monitoring well MW-14 and resample the above micro-wells at the site. The cost summary spreadsheets detailing the level of effort necessary to conduct these activities are attached.

NAM Sampling Proposal May 20, 2021

Micro-well Replacement

The last time micro-well MW-14 was sampled it contained 0.87 I,V micrograms per liter (µg/l) of

PCE, however, the well was damaged sometime after this sampling event and a sample has not been able to be collected from this well, as Wood has not been able to get a water level meter or tubing into the well. The soil under the concrete pad for MW-14 and MW-18I (co-located) has been eroded by surface water runoff from the parking lot, causing the well pad to shift and sink resulting in a bent well casing. Wood will remove the damaged pad from MW-14 and MW-18I and then re-install micro-well MW-14 to 12 feet below land surface (bls) with a 10-foot pre-packed screen. After the well is installed a new well pad will be constructed over MW-14 and

MW-18I. Micro-well MW-14 will be developed until the water is sediment-free.

NAM Groundwater Sampling Event

Wood will mobilize a two-person crew to the site to conduct the groundwater sampling activities.

Prior to collecting groundwater samples, all 20 micro-wells will be opened and allowed to equilibrate and then a synoptic round of water level measurements will be collected.

Groundwater samples will then be collected from the 16 recommended micro-wells.

Groundwater sampling will adhere to the FDEP SOP FS 2200 using low flow purging techniques with a peristaltic pump and new HDPE tubing. Field parameters including temperature, pH, specific conductance, turbidity, dissolved oxygen (DO) and oxygen reduction potential (ORP) will be measured following removal of each well volume. A minimum of three well volumes will be purged from each monitoring well. If the field parameters have stabilized (i.e., three consecutive measurements are within ranges specified in SOPs) and the water turbidity is less than 20 nephelometric turbidity units (NTU) after three well volumes, the groundwater sample will be collected. If turbidity below 20 NTU cannot be attained, the well will be purged until the turbidity measurements become stable, or once five well volumes has been purged, at which time the groundwater sample will be collected.

The groundwater samples will be analyzed for CVOCs using USEPA Method 8260B. The groundwater sample collected from source area micro-well MW-1 will be analyzed for total recoverable petroleum hydrocarbons (TRPHs) using the FL-PRO method to monitor for potential groundwater impacts from the EOS-PRO amendment that was placed in the source removal open excavation. Additionally, Wood recommends analyzing the groundwater samples collected from micro-wells MW-1, MW-5 and MW-2I for natural attenuation parameters including total iron, ferrous iron, sulfate, sulfide, nitrate, nitrite, total organic carbon. It is also recommended that Microseeps be utilized for these same three wells to be analyzed for the dissolved gases methane, ethene and ethane using USEPA Method RSK175. Samples will be shipped overnight under standard chain of custody protocol to Advanced Environmental

Laboratories (AEL) in Tallahassee, Florida.

All purge water will be containerized onsite in 55-gallon drums pending laboratory analysis. At the end of the event, an IDW sample will be collected from the drummed purge water and submitted for laboratory analysis. The drums will be staged at a secure location onsite pending the analytical results. Once the analytical results are received, the drums will be transported from the site for disposal.

Upon completion of the annual NAM sampling field activities and receipt of the analytical data, Wood will provide an Annual NAM Report detailing the activities and findings of the sampling

NAM Sampling Proposal May 20, 2021 event and will include updated tables with analytical data and groundwater elevations and figures showing analytical results, groundwater plumes and groundwater flow at the site, as well as conclusions and recommendations.
