About this file
This is a Task Assignment Notification Form (DC003K) issued under Florida Department of Environmental Protection (FDEP) Contract No. HW554 with APTIM Environmental & Infrastructure, Inc. The task assignment involves groundwater monitoring services at the Former Brantley Cleaners site located at 924 West State Road 436, Suite 1250, in Altamonte Springs, Seminole County, Florida. The contracting parties include FDEP as the state agency, with Miranda McClure serving as the acting DEP Contract Manager and Eric Kramer as the contractor representative. The task falls under FDEP's indefinite delivery contract vehicle for environmental remediation and monitoring services.
The scope of work requires APTIM to deploy replacement Passive Diffusion Bags (PDBs) into seven monitoring wells, collect groundwater depth measurements from sixteen wells, and retrieve samples for EPA Method 8260 volatile organic halocarbon (VOH) analysis. The final deliverable is a Groundwater Sampling Report with GIS tables and ADaPT electronic data deliveries (EDDs), due December 11, 2020, with task execution extending through February 12, 2021. The task assignment is structured as a fixed-price contract with a total value of $8,873.51, allocated as follows: direct labor costs of $7,113.67 (including fringe, overhead, and general and administrative expenses), laboratory analytical costs of $632.00, field equipment and expendables totaling $188.95, vehicle rental costs of $97.00, and a completion fee of $711.37. No retainage is withheld, and payment is conditioned upon FDEP approval of the deliverables, with 55 hours of total labor budgeted across multiple staff positions.

Proposed Scope of Work Annual Sampling Former Brantley Cleaners Task Assignment DC003K ERIC_5454 (Former Facility ID No. 599500158) Aptim Environmental & Infrastructure, Inc. (APTIM) 8/28/20
The former Brantley Cleaners facility, located in Altamonte Springs, is sampled on an annual basis. The proposed scope of work for Task Assignment DC003K includes the collection and sampling of seven (7) Passive Diffusion Bags (PDB’s) currently installed in wells at the site. Retrieval, sampling and replacement of the PDBs is currently due and will be scheduled upon approval of this Task Assignment.
The proposed scope of work includes the collection of depth to groundwater measurements from the 16 monitor wells (MW001 through MW016). Samples will be collected from the 7 existing monitoring wells containing the PDB bags for analysis by EPA Method 8260 for volatile organic halocarbons (VOHs). Since PDB’s are being used for this sampling event, groundwater intrinsic values will not be collected.
The monitoring wells containing PDBs include MW001, MW004, MW009, MW013, and MW016; and intermediate wells MW006 and MW012. The current PDBs were deployed into these seven wells on January 22, 2019.
Seven replacement PDB samplers will be obtained from ALS Environmental (ALS), a licensed manufacturer of passive diffusion samplers and will be placed within the wells similar to the existing samplers. The PDB samplers deployed into 2-inch diameter monitoring wells MW004, MW006, MW009, MW012, and MW013 are 24 inches long and 1 ¼ inches in diameter. The PDB samplers deployed into the smaller 1-inch diameter monitoring wells MW001 and MW016 are 48 inches long and ¾ inches in diameter. The samplers will be ordered pre-filled with approximately 220 milliliters of certified, laboratory-grade, analyte-free, deionized water. The PDB’s will be deployed similar to the current bags to the following total depths:
1) MW001, (1” well), total depth of 35 ft bls (placement from 31 to 35 ft bls)
2) MW004, (2” well) total depth of 34 ft bls (placement from 32 to 34 ft bls)
3) MW006, (2” well) total depth of 71 ft bls (placement from 69 to 71 ft bls)
4) MW009, (2” well) total depth of 51 ft bls (placement from 49 to 51 ft bls)
5) MW012, (2” well) total depth of 71 ft bls (placement from 69 to 71 ft bls)
6) MW013, (2” well) total depth of 51 ft bls (placement from 49 to 51 ft bls)
7) MW016, (1” well), total depth of 34 ft bls (placement from 30 to 34 ft bls).
An Annual Sampling Report will be provided summarizing the collected groundwater measurements and analytical results in Tables, Figures and Text including conclusions and recommendations. APTIM recommends the following schedule:
Description Due Date
Collection of GW Samples and Deployment of PDBs 9/30/20
Submit Letter Report 10/6/20
Submit GIS/Data Base Update 10/9/20
Task Expiration Date 1/30/21
APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, Inc.
and Florida Department of Environmental Protection No. HW554. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
