About this file
This document is a Subcontractor/Consultant/Supplier Disclosure Form (MBD-20) issued by the City of Tampa's Minority Business Development Office that must be submitted with all bids and proposals for City of Tampa contracts. The form requires contractors to disclose all projected subcontractors, consultants, and suppliers to be utilized on a contract, including their company information, ownership demographics, trade classifications, and contract amounts. For the Jackson Heights Gymnasium Floor Refinishing project located at 3310 E. Lake Ave. in Tampa, Florida, the selected contractor must sand, stripe, and refinish the gymnasium floor by completely sanding the surface, thoroughly cleaning and removing all foreign matter and dust, restriping the gym surface as requested by staff, and applying three even coats of oil-based MFMA-approved finish with proper ventilation. Site visit inquiries should be directed to Jamal Jefferson at the Jackson Heights NFL YET Center at (813) 753-9014 or jamal.jefferson@tampagov.net.
The form requires disclosure of total subcontract and supplier utilization amounts as well as separate calculations for Small Local Business Enterprise (SLBE) and Women/Minority Business Enterprise (WMBE) utilization, with percentages calculated against the total bid amount. Contractors must identify each subcontractor's Federal ID, ownership type (including gender and ethnicity codes), NIGP classification codes, and quote amounts, along with letters of intent for all SLBE and WMBE firms. The City of Tampa applies set-aside goals for SLBE and WMBE participation, and contractors must certify the accuracy of their subcontracting disclosures. Contractors checking the "No Subcontracting" box indicating self-performance of all work should note that certified SLBE or WMBE firms bidding as prime contractors are not exempt from outreach and solicitation requirements for subcontractors. Questions regarding the form or certification process can be directed to the City of Tampa's Minority Business Development Office at (813) 274-5522.
