This is a Conflict of Interest Questionnaire form issued by the Texas Ethics Commission for vendors conducting business with local governmental entities, specifically applicable to Collin County's cafeteria services solicitation at the Russell A. Steindam Courts Building in McKinney, Texas. The form requires vendors to disclose any employment relationships, business connections, gift transfers, or family relationships with local government officers that could present conflicts of interest. Vendors must file the completed questionnaire with the county's records administrator no later than seven business days after beginning contract negotiations, submitting a proposal, or becoming aware of a conflicting relationship. The questionnaire must be filed if the vendor has an employment or business relationship with a local government officer or family member resulting in taxable income exceeding $2,500 within the preceding 12 months, has given gifts with an aggregate value exceeding $100 to such officers within the preceding 12 months, or has a family relationship with a local government officer.

The form serves as a mandatory disclosure document under Chapter 176 of the Texas Local Government Code and applies to vendors with business relationships meeting the statutory definition of commercial activity. Vendors must certify the accuracy of their disclosures through signature and are subject to misdemeanor penalties for knowingly violating the filing requirements or providing false information. The questionnaire addresses specific relationship categories including direct employment or business income between the vendor and officers, income received by vendors from officers outside the governmental entity, relationships with entities where officers hold directorships or ownership interests of one percent or more, and any gifts provided to officers or their family members. Updated questionnaires must be filed within seven business days of discovering that originally filed information was incomplete or inaccurate.
