About this file

This is a Conflict of Interest Questionnaire form for vendors doing business with local governmental entities in Texas, as prescribed by Chapter 176 of the Local Government Code and amended by H.B. 23 of the 84th Legislative Session. The form applies to Collin County's cafeteria services solicitation for the Russell A. Steindam Courts Building in McKinney, Texas. Vendors must file the completed questionnaire with the county's records administrator no later than the seventh business day after beginning discussions or negotiations, submitting proposals, or becoming aware of conflicts of interest. The form requires vendors to disclose any employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12 months, any gifts with aggregate value exceeding $100 provided to officers or their family members within the 12-month period preceding contract awareness, and any family relationships with local government officers.

The questionnaire mandates disclosure of vendor relationships with corporations or business entities in which local government officers serve as officers, directors, or hold ownership interests of one percent or more. Vendors must also disclose any gifts provided to officers or family members, excluding certain exceptions defined in Section 176.003(a-1). Failure to file the questionnaire or knowingly providing false information constitutes a misdemeanor offense. The form specifies that "business relationship" excludes transactions subject to rate or fee regulation by governmental agencies, transactions conducted at prices and terms available to the public, and purchases or leases of goods or services from entities chartered and regularly examined by state or federal agencies. All vendors responding to the Collin County cafeteria services proposal must complete and submit this form to ensure compliance with state conflict of interest disclosure requirements.
