About this file

This is a Certification Statements form from the State of Ohio for bidders to claim preferences for Domestic Source End Products, Ohio preference, and/or Veteran Friendly Business Enterprise (VBE) status. The document outlines the requirements and certification process for bidders seeking these preferences in state contracting opportunities, specifically relating to a walking path resurfacing and culvert replacement project at the Ohio Veterans Home-Georgetown.

The form includes three main certification sections: Domestic Preference (Buy American) which requires documentation of product origin and manufacturing location; Ohio Preference (Buy Ohio) which requires verification of in-state operations and economic presence; and Veterans Preference (Buy Veteran) for certified Veteran Friendly Business Enterprises. Additionally, the form includes sections for E.D.G.E. (Encouraging Diversity, Growth and Equity) business certification and Independent Contractor acknowledgement. Any bidder who submits false information to receive preferences may be disqualified and face legal consequences including debarment.
