About this file

This is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division under the Department of Administration. The certification applies to the Rural Value Acceleration Network (RVAN) Vendor contract opportunity on behalf of the Nevada Health Authority (NVHA). The form serves as a prerequisite attestation that must be completed and submitted by vendors responding to this state and local contract solicitation. Vendors must provide the solicitation number, vendor name, authorized signer name, authorized signature, and date on the certification document.

The certification requires vendors to confirm, under penalty of perjury, that no federal appropriated funds have been or will be paid to influence federal agency officials, members of Congress, or congressional staff in connection with the award of any federal contract, grant, loan, or cooperative agreement. If non-federal funds have been or will be paid for such purposes, vendors must complete and submit Standard Form-LLL disclosing lobbying activities. Vendors must also ensure this certification language is included in all subaward documents and that all subrecipients provide corresponding certifications and disclosures. Submission of this certification is mandatory and constitutes a material representation of fact. Failure to file the required certification subjects the responsible party to civil penalties ranging from $10,000 to $100,000 for each violation, as imposed under Section 1352, U.S. Code.
