About this file

This is a cost schedule template for an RFP issued by the Nevada Health Authority (NVHA) for a Rural Value Acceleration Network (RVAN) vendor. The project encompasses seven primary scopes of work: neutral convening and facilitation, RVAN workgroup establishment and governance, rural value-based care model development, data sharing and interoperability alignment, stakeholder communication and change management, deliverables and reporting, and contractor qualifications. The contract term spans four fiscal years from March 9, 2027, through June 30, 2030, with an additional optional year one period from May 1, 2027, through June 30, 2027. Key deliverables include a detailed workplan due within 15 days of kickoff, a finalized RVAN workgroup charter due within six weeks, a rural provider readiness assessment due by week 16, monthly progress reports throughout the engagement, and a final RVAN model and governance report due no later than 15 months from contract start. The vendor must demonstrate expertise in rural value-based care, neutral facilitation among competing stakeholders, familiarity with Medicaid MCO contracting and CMS Aligned Network requirements, and provide references from similar rural value-based care facilitation projects.

The cost schedule template requires vendors to enter proposed costs across four fiscal years in columns designated for FY 2027, FY 2028, FY 2029, and FY 2030, with column J and L calculations and totals automatically formulated. Vendors must provide detailed cost methodology for each year and include all expenses encompassing travel, per diem, out-of-pocket costs, and administrative or overhead expenses, with supporting documentation for all cost schedules. All proposal terms, including proposed costs, remain valid for a minimum of 180 days following the proposal due date, and for awarded vendors, terms remain in effect throughout the entire contract period. The template indicates all costs are currently shown as $0.00, requiring vendors to populate actual proposed costs during the RFP response process.
