About this file

This document is a Certification Regarding Lobbying form for contracts, grants, and cooperative agreements, applicable to federal transactions. The certification requires the undersigned to affirm that no federal appropriated funds have been or will be paid to influence or attempt to influence congressional officers, employees, or members in connection with federal contracts, grants, loans, or cooperative agreements. If non-federal funds are used for such purposes, the entity must complete and submit Standard Form-LLL, Disclosure Form to Report Lobbying. The certification mandates that this language be included in all sub-award documents at all tiers.

The document emphasizes the legal implications of non-compliance, stating that failure to file the required certification subjects the entity to civil penalties ranging from $10,000 to $100,000 for each failure. The certification is considered a material representation of fact, and submission is a prerequisite for entering into the transaction as imposed by Section 1352, title 31, U.S. Code. The form requires signatures from the Executive Director and Chief Financial Officer, with spaces provided for the grantee's name and the title of the grant program (specifically noting NSLP or SFSP options).
