About this file

This Certificate of Partial Payment form is a standardized payment documentation instrument used by Clay County, Florida, for contractor reimbursement on the Agronomic Services for Clay County Parks contract. The certificate serves as the vendor's certification that all work has been performed and materials applied in accordance with contract terms and conditions across the 149-acre, 19-park service area. The form requires the contractor to certify that all subcontractors and suppliers have received their proportionate shares from previous progress payments and establishes strict payment forwarding timelines: contractors must remit payment to their direct subcontractors and suppliers within 10 days of receiving county payment, and subcontractors must similarly remit payment to their sub-tier vendors within 7 days thereafter. The contract term runs for one year with potential two-year extensions, with services commencing January through December and encompassing fertilization, weed control, insect and disease management, and pre-emergent applications across county park locations.

The certificate requires completion of payment number, total amount due, vendor identification, and authorized signature with date. Contractors bidding on this opportunity must have submitted proposals by the November 20, 2025 deadline, with inquiries due by November 12, 2025, and must provide a 5% bid bond. Bidders are required to self-perform at least 40% of contract work and submit comprehensive supporting documentation including W-9 forms, insurance certificates, equipment lists, personnel information, and references. Pricing is structured on a unit-cost-per-acre basis, with 135 acres designated for standard services and 27 acres for regional sportsplex services. No specific budget range or set-aside designations for disadvantaged enterprises were disclosed, though contractors must maintain drug-free workplace compliance and submit daily project reports demonstrating capability to manage comprehensive turf maintenance across multiple county locations.
