PROJECT DETAILS

State the purpose of the project and provide project details for the length of the budget period (11 months).

For the selected program strategies:

1. Describe proposed activities and provide sample process measures to track progress implementing the activities

2. Describe the expected outputs (resources, tools, products, etc.) and outcomes (change to occur), and provide sample outcome measures to track progress achieving the outcomes.

Project Purpose

This project will fund a partner organization to provide support, including funding and technical assistance, for overdose prevention efforts to community behavioral health organizations. Activities will focus on harm reduction and linkages to care. The partner organization will be expected to develop and release a request for applications that will fund established community behavioral health organizations, including harm reduction organizations, coalitions and other organizations focused on linking people who use drugs to peer support, to treatment, to recovery support, and retention in care for people who use drugs.

87/150 words maximum

Program Strategy: Programs and Services

Activities *

Through a multi-prong RFA, fund high-capacity community behavioral health organizations, including linkage to care and harm reduction organizations and/or coalitions, to provide, offer, and amplify linkage and retention in care as well as other harm reduction services to reduce overdose risk, through a health equity lens.

The funded partner organization would also use a portion of the funds to provide technical assistance/support to community behavioral health organizations as they implement activities.

CDC will work with the funded recipient to identify and prioritize the organizations that would benefit with additional funds to expand their scope and reach. Some example activities being implemented by community behavioral health organizations could be:

· Focus on disproportionately affected populations (e.g., justice-involved, homeless, people who have already experienced a drug overdose, people with disabilities, racial/ethnic minorities such as African Americans, Hispanic, Tribal populations)

· Focus on opioid, stimulant, and polysubstance use prevention

· Prioritize support to people who are “not in care”

· Potential supports could include:

o Linkage to care to (1) strengthen post-overdose outreach – via certified peer recovery specialists or navigators-- for disproportionately affected populations, and (2) support connection to wraparound services.

o Increase access to and administration of naloxone education, overdose education, and response for polysubstance use involving opioids.

o Develop and implement local media campaigns and/or promotional materials focused on reducing stigma around substance use disorder and MOUD and other evidence-based treatments, including treatments for stimulant use disorder, among the public, law enforcement, healthcare providers/clinicians.

o Increase understanding, education, and familiarity of the services and role of SSPs and other harm reduction organizations in communities—outreach and engagement of a broad array of partners o Train on use of fentanyl test strips and other strategies that can increase awareness of the illicit drug supply among people who use drugs o Scale up the adoption of the Public Health and Safety Team (PHAST) toolkit in high burden communities o Encourage community behavioral health organizations to bridge in/collaborate with local health departments in executing the work as appropriate.

Process Measures *

1. # of applications received

2. # of awards made

3. # of community behavioral health organizations funded (release of NOA)

4. # of community behavioral health organizations with executed contracts within 2 months of NOA

5. # of community behavioral health organizations implementing linkages to care strategies

7. # of community behavioral health organizations implementing harm reduction strategies

8. Types of harm reduction strategies being implemented by community behavioral health organizations (e.g., SSPs, OEND, anti-stigma campaigns)

9. Types of technical assistance provided to community behavioral health organizations

12. Topics of focus for TA

Outputs *

Project Purpose

The purpose of this project is to advance the capacity of community health centers (CHC) to prevent drug overdose due to opioids and/or stimulants through linkages to care and retention support services. The community health centers will implement existing evidence-based interventions to increase the number of people who use drugs who are receiving treatment, retention services, and to recovery support. This “linkage to care” can happen in a broad range of novel clinical settings—for example, primary care, inpatient, emergency care, and community outreach clinics. The goal is to ensure people experiencing a drug overdose are initiated on medications for opioid use disorder (if appropriate), receiving naloxone, being referred to treatment, being retained in care, connected to recovery supports at this critical time of care. This project will fund a national association to provide support, including funding and technical assistance, for overdose prevention efforts to community health centers.
