PWS – Carpet, Tile and Stair Tread Replacement/Installation
PERFORMANCE WORK STATEMENT FOR
CARPET REPLACEMENT/INSTALLATION
SERVICES
Eielson Air Force Base Alaska October 2012
PERFORMANCE WORK STATEMENT FOR CARPET, TILE AND STAIR TREAD
REPLACEMENT/INSTALLATION
1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with these specifications. Work may include installing and or replacing: base, carpet, vinyl composition tile, rubber stair treads and risers, rubber tile, ceramic tile, outdoor matting; patching floor, and providing labor to remedy unusual requirements related to the installation or replacement of flooring materials. All flooring material to include carpet, vinyl composition tile, and rubber stair treads and risers, rubber tile and ceramic tile will be Government furnished. The required work will be on an as needed basis and at various locations on Eielson Air Force Base, AK. This general outline of work does not, in any way, limit the responsibility of the Contractor to perform all work as specified herein.
1.1 WORK INCLUDED:
1.1.1 Remove existing cove or wood base and floor or step covering. Dispose of rubbish off site/base.
1.1.2 Prepare and measure all surfaces to receive new flooring.
1.1.3 Install stretch carpet or glue down flooring on surfaces where indicated.
1.1.4 Install cove, carpet or wood base as directed.
1.1.5 Install Contractor furnished edge strips where flooring terminates at other floor finishes.
1.1.6 Surge carpet as required.
1.1.7 Once a job is started, the Contractor is expected to finish that job without delay, unless directed by the QAE.
Weekends and Public Holidays are considered acceptable delays.
1.1.8 If the Contractor experiences a delay due to the Using Agency or by means outside the Contractors control, the Contractor shall advise the QAE as soon as possible. The QAE will log the delay, and if possible, clean up the delay.
1.1.9 The Contractor will dispose of all scraps after installation and vacuum or sweep the entire area where flooring was installed. Dispose of remnants as directed by the QAE.
1.1.10 Upon completion of installation the contractor will promptly notify the QAE for final inspection and acceptance of service.
1.1.11 The Contractor will experience a 15-20 minute delay entering and exiting controlled facilities.
1.1.12 Contractor will include installing Collins and Aikman carpet, which will require installation certification from the company.
1.2 WORK EXCLUDED:
1.2.1 1.2.1. All furniture and equipment will be moved by the Using Agency.
1.3 REFERENCE STANDARDS:
All work shall be accomplished in accordance with CRI-104, Standard for Installation of Commercial Textile Floor Covering Materials and the Manufacturers Specifications.
2 PRODUCTS:
2.1 Adhesives and Primers. Adhesives and primers shall be water resistant type and consistency recommended by the manufacturer of the materials to be installed for the type and conditions of the surfaces to be covered.
Adhesives shall be used in strict accordance with manufacturer's printed instructions.
2.2 Edge Molding. Edge molding shall be heavy duty vinyl designed for the type of flooring installed. Floor flange shall be minimum 2" wide. Color shall match base or as specified by the QAE.
2.3 Rubber Base. After flooring is installed, the Contractor shall install set-on type, rolled, rubber base 1/8 inch thick and in height (either 4 inches or 6 inches) indicated. An adequate coved backing strip to prevent "poke-thru" shall be provided. Color to be specified by the QAE.
2.4 Seam adhesive shall be waterproof, nonflammable and non-staining as recommended by the carpet manufacturer.
2.5 Patching Material: Repair holes, cracks, depressions or rough areas using material recommended by the flooring or adhesive manufacturer.
3 EXECUTION:
3.1 The Civil Engineering, Functional Area Chief, shall appoint QAE’s to monitor and inspect the BPA. The QAE shall notify the Contractor to schedule the installation of flooring.
3.2 If the possibility exists for Asbestos containing material (ACM) in the area to be worked it will be identified prior to any work. If there is ACM in the area the Contractor will sign a statement stating they will not use any mechanical means to remove the ACM. In the event during carpet removal it is determined that the ACM must be removed the Contractor must stop and notify the QAE. Removal of ACM is accomplished by the Government through the use of a separate Asbestos Removal Team.
3.3 PREPARATION OF SURFACES:
3.3.1 Inspect all areas before installation. Repair holes, cracks, depressions or rough areas using material recommended by the flooring or adhesive manufacturer. Floors shall be free of any foreign materials and swept broom clean. Minimum floor temperature before, during and after installation and requirements for conditioning adhesive shall comply with the carpet and adhesive manufacturer's instructions; however, in no case shall floor temperature be less than 60 degrees F. for 24 hours prior to, during and after installation.
3.4 INSTALLATION:
3.4.1 Check matching of carpet before cutting and ensure there is no visible variation between dye lots. Also check for any other manufacturing defects prior to installation. Do not install defective carpet.
3.4.2 Cut carpet, where required, in a manner to allow proper seam and pattern match. Ensure cuts are straight and true and unfrayed.
3.4.3 Where possible and practical, locate seams in areas of least amount of traffic. Seams shall occur only where necessary due to carpet width. No cross seams will be accepted.
3.4.4 Join seams in recommended manner so as not to detract from the appearance of the carpet installation and decrease its life expectancy. Ensure seams are straight, not overlapped or peaked and free of gaps.
3.4.5 Vacuum clean substrate. Spread adhesive in quantity recommended by manufacturer to ensure proper adhesion over full area of installation. Apply only enough adhesive to permit proper adhesion of carpet before initial set.
3.4.6 Lay carpet on floor with the run of the pile in the same direction of anticipated traffic.
3.4.7 Do not change run of pile in any one room or from one room to next where continuous through a wall opening, unless directed by the QAE.
3.4.8 Cut and fit carpet neatly around projections through floor and to walls and other vertical surfaces.
3.4.9 Fit carpet snugly to walls or other vertical surfaces where no base is scheduled, leaving no gaps.
3.4.10 Do not place heavy objects such as furniture on carpeted surfaces for minimum of 8 hours or until adhesive is set.
3.4.11 Entire carpet installation is to be laid tight and flat to sub floor, well fastened at edges, to present a uniform pleasing appearance. Ensure monolithic color, pattern, and texture match within any one area.
3.4.12 Install edging strips where carpet terminates at other floor coverings. Use full-length pieces only. Butt tight to vertical surfaces. Where splicing cannot be avoided, butt ends tight and flush.
3.4.13 Install straight base where indicated.
4.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES
4.1. Availability of utilities. All reasonable quantities of utilities shall be made available to the contractor without cost. Any temporary connection of lines that may be required and removed by the Contractor will be at the Contractors expense. Temporary connection shall follow existing lines of the building and shall be installed safely IAW standard commercial practices.
4.2. Storage of material. The Contractor shall not store materials and equipment in the work area without approval and coordination with 354 CES/CEOSS. Material and equipment approved for storage shall be placed to provide continued facility use, neat in appearance and shall assure safety.
5.0 GENERAL INFORMATION
5.1 Hours of Operation. Work shall be performed in a manner to create minimum disturbance and inconvenience to the use of the facility. Contractor personnel will be limited to work areas. Personnel clearances, access or escorted entry shall be coordinated with 354 CES/CEOSS Service Contracts, (907) 377-2595.
5.2 Recognized Holidays. The Contractor is not required to provide service on Federal Holidays, except in emergency situations, the holidays are:
• New Year’s Day - 1 January
• Martin Luther King Day - 3rd Monday in January
• Washington’s Birthday - 3rd Monday in February
• Memorial Day - last Monday in May
• Independence Day - 4 July
• Labor Day - 1st Monday in September
• Columbus Day - 2nd Monday in October
• Veteran’s Day - 11 November
• Thanksgiving Day - 4th Thursday in November
• Christmas Day - 25 December
5.4 Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
5.5 Security Requirements
5.5.1 DoD Identification Card. Eielson Air Force Base is a Closed Base. All personnel entering must have specific permission of the installation commander for entry. This permission is granted when the Contractor and employees are issued a Contractor Identification Card. Each prime Contractor will complete a listing of all personnel needing regular access to the base in association with this contract within 15 calendar days following the contract award date. The following items required are as follows:

Employee’s Social Security Number Vehicle Color Vehicle License Number Vehicle Identification Number This is required of all personnel and must be kept current as changes occur.
After review and approval, which may take up to 5 working days, those listed personnel being authorized entry to the base will proceed to Pass and Registration for issuance of their civilian employee identification card.
4.5.2 Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor’s employees shall be briefed by their immediate supervisor upon initial on-base assignment.
4.5.3 Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
4.6 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, and requirements regarding environmental protection to include AFI 32-7080, Pollution Prevention Program, AFI 32-7042, Solid and Hazardous Waste Compliance, and ACC Manual 32-7051, Environmental Quality Manual, and 40 CFR 273, 40 CFR 133, North Dakota Administrative Code 33-24-02-06, and base Hazardous Waste Management Plan (September 2005) 5 BW-32-7043. In the event environmental laws/regulations/requirements change during the term of this contract, the Contractor shall comply with such laws/regulations as changed. The Contractor shall comply with the base Hazardous Waste Management Plan (September 2005) when turning in wastes generated from spill cleanup.
4.7 Notification of Environmental Spills. If the Contractor spills or releases any substance listed in 40 CFR 302, Designation, Reportable Quantities, and Notification or general fluids into the environment, the Contractor shall immediately report the incident to the QAP, and be familiar with and follow the most current "Full Spectrum Threat Response (FSTR) Plan 10-2" (a copy will be provided at the pre-performance meeting). The
Contractor shall be held liable for the spill or release of such substances, and is expected to clean up all spills of less than 25 gallons. The Contractor is responsible for advising his employees of all Environmental and Hazardous Materials Handling and is also required to have and maintain Material Safety Data Sheets (MSDS) on all materials that are required by state and federal laws and/or regulations (reference AFI 91-301 and OSHA requirements).
Copies of MSDSs must be provided to CEV through the QAP for all materials prior to being brought on Minot Air Force Base. Usage and storage location of these materials must be provided quarterly to CEV through the QAP.
4.8 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.
4.9 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately owned vehicle while on Minot Air Force Base.
4.10 For Official Use Only (FOUO). The Contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.11 Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
The Government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well being, or operational mission of the installation and its population.
The Contractor shall ensure employees have the current and valid professional licensing/certifications to accomplish the services found in this SOW before starting work under this contract and are in compliance with all federal, state, and local environmental requirements or laws.
Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements in maintenance of elevators. All work shall be accomplished by workmen skilled in the trade of inspection, maintenance, service and repair of elevators with a minimum of 5 years experience. Prior to any cutting or open flames, an AF Form 592 welding permit shall be obtained from the base Fire Dept. 320 Flightline Drive. Fire Dept. 701-723-2461
4.12 Damage to Government Property. The Contractor shall notify the government when damage to government property is discovered. Any damage to government property caused by the Contractor shall be repaired or replaced by the Contractor at the Contractor’s expense. The Contractor shall annotate on the inspection report, the damage, the cause of damage (if known) and a proposal for repair.
4.13 Fire and Emergency Reporting. Dial 911 from a standard phone. Dial 723-2461 from a cellular phone.
