About this file
This file is a product specification image for canvas tote bags with the Maine logo, submitted in response to a request for quotes (RQS1039) issued by the State of Maine, Division of Purchases. The solicitation requested quotes on canvas tote bags featuring the Maine logo, with responses due by December 7, 2009. The State indicated that awards would be made to the lowest responsible bidder based on best value considerations, including quality, conformity with specifications, delivery date, and ultimate cost. In the event of tie bids, preference would be given to in-state bidders or those offering commodities produced or manufactured in Maine. Bidders were required to propose unit prices and could offer approved equal alternatives to specified products with appropriate documentation.
All bidders were required to submit a notarized State Purchasing Code of Conduct Affidavit identifying the complete physical locations of all plants where goods would be assembled, including manufacturer plant names, street addresses, cities, states, countries, and telephone numbers. Vendors and their suppliers at the point of assembly were required to certify compliance with all applicable wage, health, labor, environmental, safety, building, and fire codes, as well as laws prohibiting discrimination in hiring, promotion, or compensation. Additionally, vendors were required to certify compliance with human and labor rights treaty obligations prohibiting forced labor, indentured labor, slave labor, child labor, involuntary prison labor, physical and sexual abuse, and restrictions on freedom of association. The affidavit had to be faxed to Sue Garcia at 207-287-6578 no later than the bid opening date and time. Cash discounts of 20 days or more were considered in bid evaluation, while discounts under 20 days would not be considered. The State was exempt from Federal and Maine State Sales and Use Taxes, and all transportation charges, including freight and express costs, were to be prepaid by the contractor unless otherwise specified.
