About this file
This is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division, Department of Administration, on behalf of the Nevada Physical Therapy Board in connection with a Request for Proposals (RFP) for government relations services. The certification document establishes the lobbying compliance requirements that vendors must acknowledge and certify to in order to submit proposals for the government affairs contract.
The form requires authorized signers to certify that no Federal appropriated funds have been or will be paid to influence officers or employees of any agency, Congress members, or congressional staff in connection with the Federal contract award. Vendors must also certify that if non-Federal funds are used for lobbying activities related to the contract, they will complete and submit Standard Form-LLL, "Disclosure of Lobbying Activities," in accordance with applicable instructions. Additionally, vendors must ensure that this certification language is included in all sub awards at all tiers and that all sub recipients certify and disclose accordingly. The certification is a material representation of fact required by section 1352, U.S. Code, and submission is a prerequisite for contract award. Failure to file the required certification subjects the vendor to civil penalties ranging from $10,000 to $100,000 for each failure.
