About this file
This is an Evaluation Determination document from the Arizona Department of Veterans' Services (ADVS), issued under Governor Douglas A. Ducey's administration and Director Wanda Wright's leadership. The document details the results of a Request for Proposal (RFP) No. BPM004543 for Pharmaceutical Services for the Arizona State Veterans' Homes. Three companies submitted proposals: Guardian Pharmacy of Arizona dba Saliba's Extended Care Pharmacy, LCPS Acquisition, LLC d/b/a Omnicare of Chandler, Neighborcare, and Pharmacy Corporation of America dba PharMerica.
After an evaluation conducted on July 5, 2022, by a committee using a 1,000-point scale and examining criteria including experience, capacity, and proposed approach, Guardian Pharmacy of Arizona dba Saliba's Extended Care Pharmacy was selected as the most advantageous offeror. The evaluation excluded cost scores for the proposals from LCPS Acquisition and PharMerica after a "not susceptible" determination was made during the initial evaluation process. The contract was awarded pursuant to Arizona Revised Statutes §41-2534 and §41-2535, with the determination effective and remaining in place unless modified or revoked.
