About this file
This document is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees. It states that contractors and grantees must certify compliance with various Pennsylvania labor and workforce safety laws, including the Unemployment Compensation Law, Workers' Compensation Law, and other applicable state labor laws. The form requires the contractor/grantee to certify that they are compliant and understand that providing false information can result in contract termination and three-year ineligibility to bid on state contracts.
The related state and local contract opportunity is for the Department of Labor and Industry (DLI) to award multiple backup contracts for on-site and off-site document destruction (shredding) services to be utilized when the statewide contractor is unable to provide the required services. The contractor shall provide all labor, equipment, materials, supplies, and transportation necessary to complete the document destruction. This is a supplemental bid to an original bid, and DGS Certified Small Businesses are encouraged to participate.
