About this file

This is a Single Source Justification document for a simplified acquisition under the Simplified Acquisition Threshold (SAT) from the VA Pittsburgh Healthcare System. The justification is for the purchase of six Invacare motorized ISAXPLUS sit-to-stand patient lifts for the Altoona VA Medical Center.

The document explains that these specific lifts are required for consistency across the Altoona VAMC and its five Community Based Outpatient Clinic locations, where patients and caregivers receive training on the equipment. The lifts were selected for their lift capacity, base opening, and multiple operating modes. Market research was conducted through the Supply Chain Master Catalog, National Strategic Source Listing, and GSA FSS contract, with no mandatory sources found available. The justification was signed by Contracting Officer Keri Hester on December 31, 2024.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

Revision: 01 Effective Date: 11/15/2024 Page 1 of 1 Acquisition ID#:

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106 1 For

Over Micro Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C244 25 AP 0638

1. Contracting Activity: Department of Veterans Affairs, VISN 4, VA Pittsburgh Healthcare System, Pittsburgh, PA 15215. 2237# 503 25 1 225 0040..

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Altoona VA Medical Center has a brand name requirement for six (6) Invacare motorized ISAXPLUS sit to stand patient lifts.

The estimated cost of this procurement is:

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Altoona VAMC prosthetics currently ships Invacare motorized ISAXPLUS to their patients managed by occupational therapy. These lifts were chosen for their lift capacity, base opening, and multiple operating modes. Due to the patients and caregivers receiving evalution and instructions for use either the Altoona VAMC or one of the five (5) Community Based Outpatient Clinic (CBOC) locations, it is imperative the occupational therapy locations have the same model patient lift as the patients will receive for use in their homes. This consistency will ensure the highest safety and success rates for the patients.

4. Description of market research conducted and results or statement why it was not conducted:

The CS searched the Supply Chain Master Catelog and the National Strategic Source Listing for mandatory sources. The CS also searched GSA for an FSS contract. There were no mandatory sources available.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106 1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
