About this file
The Bidder's Questionnaire is a procurement document for the City of Napa's Fire Station No. 3 Driveway Repair Project, designed to qualify potential contractors for the construction work. The document requires bidders to provide comprehensive details about their business, including years in operation, past project experience, safety records, and potential disqualifying events. Bidders must submit the completed questionnaire within 48 hours of the City's request, typically following the bid opening, and the document serves as a critical evaluation tool for assessing contractor qualifications for the specific public works project.
The questionnaire requires extensive documentation, including the bidder's business structure, contractor license numbers, past project details, and safety performance metrics. Contractors must disclose their Experience Modification Rate (EMR), work-related injury statistics, and any historical incidents involving project terminations, legal violations, or safety citations. The document emphasizes transparency, requiring bidders to provide detailed information about their six most recently completed public works projects, three largest projects, and any projects similar in scope to the Fire Station No. 3 Driveway Repair Project. The final section includes a verification component where the bidder must declare under penalty of perjury that all provided information is accurate and complete.
