The document provides a comprehensive list of documents that potential bidders must submit with their bid package, including a Bidder's Checklist, Bidder Certification, Bid Rate Sheet, Payee Data Record, and various other required certifications and declarations.
The checklist emphasizes the importance of consistent company naming across all submitted documents, specifically matching the name on the Payee Data Record (STD 204). Bidders must provide documentation such as business licenses, incorporation documents, or good standing affidavits, and complete various state-required certifications including Commercially Useful Function (CUF), Disabled Veteran Business Enterprise (DVBE) declarations, and compliance with California Civil Rights Laws. The document also includes a link to the Contractor Certification Clauses and requires submission of a sample invoice, with a warning that failure to submit documents as prescribed may delay the agreement award.
