About this file
This is a Bid Submittal Form for a Job Order Contract (JOC) issued by Pinellas County, Florida, related to the Fiscal Year 2027 Pavement Preservation Package 3 consisting of four paving locations totaling approximately 38.92 lane miles of roadway. The contract is structured as an indefinite quantity contract requiring bidders to submit adjustment factors that will be applied to unit prices in the Construction Task Catalog rather than individual line-item pricing. Bidders must provide a single adjustment factor for each of five work categories: Non-Secure Areas with Normal Working Hours, Non-Secure Areas with Other than Normal Working Hours, Secure Areas with Normal Working Hours, Secure Areas with Other than Normal Working Hours, and Non Pre-priced Tasks. All adjustment factors must be specified to four decimal places using conventional rounding methodology.
The bid evaluation employs a weighted distribution system to calculate an Award Criteria Figure, with the work categories weighted as follows: Non-Secure Areas, Normal Working Hours at 60 percent; Non-Secure Areas, Other than Normal Working Hours at 10 percent; Secure Areas, Normal Working Hours at 10 percent; Secure Areas, Other than Normal Working Hours at 10 percent; and Non Pre-priced Work Tasks at 10 percent. Specific pricing constraints apply, including requirements that adjustment factors for other-than-normal working hours must equal or exceed those for normal working hours within the same area classification, that secure area adjustment factors must equal or exceed non-secure area adjustment factors for the same time period, and that non pre-priced work adjustment factors must equal or exceed 1.0000. The County reserves the right to correct arithmetic errors and clarifies that the weighted percentages used for bid evaluation do not represent the actual distribution of work orders to be issued under the contract.

ANY CORRECTIONS TO BIDDER ENTRIES SHALL BE INITIALED BY BIDDER.
TO THE COUNTY OF PINELLAS, FLORIDA:
We, the undersigned, hereby declare that no person or persons, firm or corporation, other than the undersigned, are interested in this proposal, as principals, and that this Proposal is made without collusion with any person, firm or corporation, and we have carefully and to our full satisfaction examined the Contract Documents, and that we have made a full examination of the location of the proposed work and the source of supply of materials, and we hereby agree to furnish all necessary labor, equipment, and materials, fully understanding that this is an indefinite quantity Contract, and that we will fully complete all necessary work in accordance with the Contract Documents, within the time limit specified in each Job Order for the unit prices set forth in the Construction Task Catalog®, multiplied by the Adjustment Factors inserted below.
All Unit Prices listed in the Construction Task Catalog® are priced at a net value of 1.0000. The Adjustment Factors shall be an increase or decrease to all the Unit Prices listed in the Construction Task Catalog®. For example, 1.1000 would be a 10% increase to the Unit Prices and 0.9500 would be a 5% decrease to the Unit Prices. Bidders who submit separate Adjustment Factors for separate Unit Prices will be considered non-responsive and their bid will be rejected.
EXAMPLE: Write the Adjustment Factor to four decimal places as the following example illustrates.
