Each Seller employee working in a City facility shall personally sign in and sign out using their full first and last name and the actual time of entry and exit. Employees shall not sign in or out for another employee. Incomplete, inaccurate, falsified or unidentified sign-in entries shall constitute a performance deficiency.
A minimum deduction of $25.00 per occurrence may be assessed for each incomplete, inaccurate, falsified or unidentified entry. In addition, any entry that prevents verification of an employee’s actual presence in the facility may be counted as skipped service, with corrective action taken in accordance with the Sellers’ Performance provisions of this Agreement.
ADDITIONAL SERVICES REQUEST
An hourly rate will be established for all other work requested of the Seller by the City in keeping with typical janitorial and cleaning services such as but not limited to additional deep cleaning when a room changes function such as moving from storage to office use or spruce up cleaning for visitors, etc. All additional service projects must be quoted and approved by the City Representative prior to work commencing.
The Seller will be reimbursed by the City on the basis of the hourly labor rate specified by the Seller in Exhibit A Item 2. of this Agreement, plus the City of Tulsa approved cost of the materials needed for the additional cleaning service.

EMERGENCY AND/OR PANDEMIC SERVICES
In the event of an emergency and/or pandemic threat to the City of Tulsa, the City Representative will work closely with the Seller to ensure appropriate Centers for Disease Control (CDC) disinfectants are utilized, and any additional Janitorial Services requirements are met. The frequency of cleaning and disinfecting will be required to be recorded on the work report checklist and provided daily to the City Representative. Checklist list will be provided. Failure to provide this information as required will result in the City of Tulsa taking the appropriate actions necessary to ensure these guidelines are met.
SELLERS’ PERFORMANCE AND PENALTIES
